This position acts as a navigator for patients through the appropriate levels of behavioral health care based on their needs.</p>\n<p> </p>\n<p><em>S<strong>ignet </strong><strong>Health is credentialed through the American Nursing Credentialing Center (ANCC) for CEUs.</strong></em></p>\n<p> </p>\n<p><strong>Key Responsibilities:</strong></p>\n<p> </p>\n<ul>\n <li><strong>Intake:</strong> Respond promptly to inquiries, assess admission eligibility, determine level of care, and manage referrals.</li>\n <li><strong>Clinical Coordination:</strong> Review referrals, assign cases, and support patient flow across levels of care.</li>\n <li><strong>Patient Support:</strong> Coordinate specialty supplies for patient needs.</li>\n <li><strong>Crisis Management:</strong> Handle urgent situations and connect patients to emergency services as needed.</li>\n <li><strong>Communication:</strong> Serve as liaison between patients, providers, and referral sources with strong customer service.</li>\n</ul>\n<h2>Requirements/Qualifications</h2>\n<p><strong>Requirements and Qualifications:</strong></p>\n<p> </p>\n<ul>\n <li>RN license required, BSN is preferred.  </li>\n <li>Related experience in a behavioral health setting with at least two of those years on an inpatient unit.</li>\n <li>Thorough understanding of Trauma Informed Care.</li>\n <li>Strong communication, interpersonal, organizational, and computer skills (EHR systems, Microsoft Office).</li>\n <li>Ability to handle stress, maintain confidentiality (HIPAA), and demonstrate empathy for clients.</li>\n</ul>",
