We hire exceptional individuals seeking meaningful opportunities in a purpose-driven environment.</p><p style=\"margin-left:0in;\"><strong>JOB SUMMARY</strong></p><p style=\"margin-left:0in;\">Record vital signs, report adverse medication/treatment reactions, and administer injections as prescribed. Assess symptomology and provide crisis intervention and medication education.</p><p style=\"margin-left:0in;\"><strong>RESPONSIBILITIES</strong></p><ul><li>Reasonable accommodation may be made for individuals with disabilities to perform essential job functions.</li><li>Provide injections, medication support, diagnostic reviews, case consult, crisis intervention, and targeted case management services to assist clients with basic needs and symptom management.</li><li>Vitals, health and wellness screenings/assessments, wellness coaching and education.</li><li>Accurate and timely documentation as contractually required and meet minimum billable requirements for direct client services.  </li><li>Dispense and account for prescribed medication, assist psychiatrist in administering and evaluating prescribed medication and arrange lab work according to protocol. Ensure doctor&rsquo;s orders are carried out and coordinate medication orders and refills with pharmacies.</li><li>Shift coverage including part of the on-call team. </li><li>Provide field-based services as required by program and client needs. </li><li>Maintain confidentiality of client care. </li><li>Participate in scheduled program meetings. Attend Quarterly Medication Monitoring Committee.</li><li>Other relevant duties as assigned. </li></ul><p style=\"margin-left:0in;\"><strong>QUALIFICATIONS</strong></p><div style=\"margin-left:0in;\"><ul style=\"list-style-type: disc;\"><li style=\"margin-left:0in;\">Licensed Vocational Nurse in California is required.</li><li style=\"margin-left:0in;\">Valid CA Driver&rsquo;s License and current auto insurance per contract requirement required.</li><li style=\"margin-left:0in;\">Experience providing mental health and/or substance use disorder services preferred.  </li><li style=\"margin-left:0in;\">Cultural competency understanding is required. Bilingual preferred. </li><li style=\"margin-left:0in;\">Knowledge of Microsoft Office and EHR software preferred. </li></ul></div><p style=\"margin-left:0in;\"><strong>PHYSICAL REQUIREMENTS</strong></p><p style=\"margin-left:0in;\">These physical demands represent what must be met to successfully perform the essential functions of this position.
