    "description": "<h2>Position Overview</h2>\n<ul>\n <li><strong>Department:</strong> Case Management</li>\n <li><strong>Schedule:</strong> Monday-Friday 8am-4:30pm with rotating weekends every 9-10 weeks</li>\n <li><strong>Work Location:</strong> UAB St. Vincent's Birmingham</li>\n <li><strong>Benefits for eligible positions only include:</strong> generous paid time off, paid parental leave, Associate Assistance Program, Tuition Reimbursement Program, and more </li>\n</ul>\n<h2>What You Will Do</h2>\n<p>Coordinates the overall interdisciplinary plan of care for patient, from admission to discharge.</p>\n<ul>\n <li>Creates plan for care across the continuum, integrating patient/family preferences and values.</li>\n <li>Monitors patient medical necessity and level of care through assessments, ongoing evaluations and/or patient records.</li>\n <li>Advocates for resources and removal of barriers.</li>\n <li>Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.</li>\n <li>Acts as a resource for adequate medical record documentation, level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care.</li>\n</ul>\n<h2>What You Will Need</h2>\n<p><strong>Licenses / Certifications / Registrations</strong></p>\n<ul>\n <li>BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.
