In this role, you will build strong relationships with physicians, hospitals, and community providers, communicate patient care needs, manage account relationships, educate referral sources on service capabilities, and collaborate with internal agency staff. If you’re ready to make a meaningful impact in a supportive, mission-driven environment, apply today!</p>\n<p> </p>\n<p>***This role requires an active nursing license, with an LPN or RN preferred***</p>\n<p> </p>\n<p><strong>Office Location: </strong>Knoxville, TN<strong>Coverage area:</strong><strong> </strong>Knoxville, TN</p>\n<p><strong>Schedule:</strong> Full-Time; Days</p>\n<ul>\n <li>Monday - Friday 8:00am - 5:00pm</li>\n</ul>\n<p> </p>\n<p><strong><em>How YOU will benefit:</em></strong>  </p>\n<ul>\n <li>Provide 1:1 care to make a lasting impact on patients and families </li>\n <li>Greater work/life balance with flexible scheduling options </li>\n <li>Less time on your feet compared to other settings </li>\n <li>Ability to work independently while also having team support </li>\n <li>Job stability and regular advancement opportunities with a growing company</li>\n</ul>\n<p><strong><em>Benefits and Perks for You!</em></strong><strong> </strong>  </p>\n<ul>\n <li>Medical, Dental, Vision insurance  </li>\n <li>Health Savings & Flexible Spending Accounts (up to $5,000 for childcare)  </li>\n <li>Tuition discounts & reimbursement  </li>\n <li>401(k) with company match  </li>\n <li>Mileage Reimbursement</li>\n <li>Generous PTO  </li>\n <li>Access to wellness and discount programs such as Noom, SkinIO (Virtual Skin Cancer Screening), childcare, gym memberships, pet insurance, travel and entertainment discounts and more!  </li>\n</ul>\n<p> </p>\n<p>*Benefits may vary by employment status </p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Manage communication between physicians, social workers, discharge planners, hospital case managers, and agency staff regarding referral requests and concerns</li>\n <li>Relay patient care information, inquiries, and status updates to referral sources</li>\n <li>Maintain a system for tracking non-admits and communicate outcomes to referral sources</li>\n <li>Coordinate care needs and communicate physician and staff concerns prior to admission or resumption of care</li>\n <li>Facilitate clinical orders from facilities or physicians and communicate requirements to agency staff</li>\n <li>Build and sustain collaborative relationships with facility and physician staff</li>\n <li>Facilitate patient referrals for home health and skilled private duty services prior to discharge</li>\n <li>Conduct pre-discharge facility visits at physician request to assess patient needs and eligibility</li>\n <li>Notify primary care providers or specialists of patient discharge and upcoming home health admission</li>\n <li>Manage assigned accounts and identify opportunities for clinical alignment to enhance outcomes</li>\n <li>Educate clinical providers on home health and skilled private duty service capabilities</li>\n <li>Communicate patient acceptance and document required demographic, clinical, and payer information on approved forms</li>\n <li>Support timely processing of physician orders at the branch level</li>\n <li>Promote effective communication and collaboration with branch and sales staff through weekly meetings</li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>Licensed Practical Nurse (LPN), Registered Nurse (RN), Certified Respiratory Therapist (CRT), or Registered Respiratory Therapist (RRT) licensure in the applicable state preferred. </li>\n <li>Reliable personal transportation, valid driver’s license, and current automobile insurance required.</li>\n</ul>\n<h2>About our Line of Business</h2>Adoration Health, an affiliate of BrightSpring Health Services, provides quality and compassionate services in the comfort of home, providing support for patients, families, and caregivers in their time of need.
