Description Position Summary The individual provides pharmaceutical care services to the patients including, but not limited to, oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy. Also, provides drug information, patient information and oversight of information and dispensing systems and ensures compliance with Federal, State, and local laws and regulations. May also advise physicians on medication utilization. Responsibilities This individual’s responsibilities include (but are not limited to):  Review and monitor patient's medication therapy and orders written for safety and efficacy considering specific patient conditions and health status;  Advise patient care team on patient's medication therapy and document such interventions.  Order verification and performing medication profile reviews to include, but not limited to; checking for duplications, drug- drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency.  Prepare, label, and dispense medications for patients in accordance with licensed independent practitioner to ensure compliance with Federal, State, and local laws and regulations pertaining to the dispensing of drugs and controlled substances and the maintenance of required records.  Check High Risk IVs. Renal dose adjusting medications. IV to PO conversions. Initiates / modifies / discontinues therapy Mediates black box warning interventions.  Ensure appropriate medications are available for patients and properly procured, prepared, stored, and maintained.  Supervise the preparation, maintenance, and dispensing process of drugs and supplies, including technical personnel performing these functions.  Ensure all medication is checked before leaving the pharmacy. Maintain relationships with medical and nursing staff; provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission standards.  Provide for collaborative practice management as per facility-approved protocols for anticoagulation, parenteral nutrition, vancomycin, and aminoglycoside consults, etc.  Assist with medication reconciliation and clarification of unsafe and unclear drug therapy orders.  Provide education to patients on the use of medications as needed. Participate in the education and training of healthcare professionals (e.g. physicians, nurses, pharmacy technicians, and students) in relation to medication use.  Demonstrate regulatory compliance as evidenced by timely completion of nursing unit inspections, as needed.  Maintain professional competence with pharmacy department and state licensing requirements via staff development, independent study, and/or professional activities. Completes monthly perpetual inventory checks as assigned. Completes area inspections as assigned in the time frame assigned. Provides on-call coverage as assigned as outlined in Department Policy. Acts under the supervision of facility DEA registrant or facility physician lead while performing medication management related activities at a remote site commonly owned and operated by the Hospital Organization/Work Role: Assesses and prioritizes a rapidly changing workload. Completes work in a timely manner. Maintains a clean and orderly work environment. Shares responsibility for ordering and stocking medication and keeping the department clean. Utilizes personal protective equipment to protect from exposure to chemical, radiation and infectious hazards. Professional: Participates in department management by providing input at meetings and reading communication. Mentors new and less experienced staff and students. Adjusts work schedule to departmental needs. Uses policy and procedure manuals and other reference material to ensure proper course of action. Quality Improvement: Identifies, proposes solutions and assists in implementation of ideas for improvement in patient care and departmental / hospital operations. Participates in the successful implementation of change. Identifies opportunities and implements strategies to decrease waste in medication. Participates in adverse drug reaction reporting and drug utilization review. Participates in department QI as requested. Other related job tasks or responsibilities as assigned Qualifications Minimum Requirements Education: Graduate of Accredited School of Pharmacy. Licensure/Certifications: Current Pharmacist license in the State of Arizona. Special Skills:  Must be able to work in a fast-paced environment with occasional periods of above average pressure.  Must have basic computer skills.  Excellent customer service skills.  Must be willing to work in all areas of the pharmacy services as assigned.  Effective interpersonal skills and ability to work with team members required.  Must be able to work in a fast-paced environment with occasional periods of above average pressure. Preferred Requirements: Experience: Prefer one (1) year of acute care experience but will consider new graduate.     "InternalResponsibilitiesStr": "<ul><li>This individual’s responsibilities include (but are not limited to): <ul><li>Review and monitor patient's medication therapy and orders written for safety and efficacy considering specific patient conditions and health status; </li><li>Advise patient care team on patient's medication therapy and document such interventions. </li><li>Order verification and performing medication profile reviews to include, but not limited to; checking for duplications, drug- drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency. </li><li>Prepare, label, and dispense medications for patients in accordance with licensed independent practitioner to ensure compliance with Federal, State, and local laws and regulations pertaining to the dispensing of drugs and controlled substances and the maintenance of required records. </li><li>Check High Risk IVs.</li><li>Renal dose adjusting medications.</li><li>IV to PO conversions.</li><li>Initiates / modifies / discontinues therapy</li><li>Mediates black box warning interventions. </li><li>Ensure appropriate medications are available for patients and properly procured, prepared, stored, and maintained. </li><li>Supervise the preparation, maintenance, and dispensing process of drugs and supplies, including technical personnel performing these functions. </li><li>Ensure all medication is checked before leaving the pharmacy.</li><li>Maintain relationships with medical and nursing staff; provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission standards. </li><li>Provide for collaborative practice management as per facility-approved protocols for anticoagulation, parenteral nutrition, vancomycin, and aminoglycoside consults, etc. </li><li>Assist with medication reconciliation and clarification of unsafe and unclear drug therapy orders. </li><li>Provide education to patients on the use of medications as needed.</li><li>Participate in the education and training of healthcare professionals (e.g. physicians, nurses, pharmacy technicians, and students) in relation to medication use. </li><li>Demonstrate regulatory compliance as evidenced by timely completion of nursing unit inspections, as needed. </li><li>Maintain professional competence with pharmacy department and state licensing requirements via staff development, independent study, and/or professional activities.</li><li>Completes monthly perpetual inventory checks as assigned.</li><li>Completes area inspections as assigned in the time frame assigned.</li><li>Provides on-call coverage as assigned as outlined in Department Policy.</li><li>Acts under the supervision of facility DEA registrant or facility physician lead while performing medication management related activities at a remote site commonly owned and operated by the Hospital</li></ul></li><li>Organization/Work Role:<ul><li>Assesses and prioritizes a rapidly changing workload. Completes work in a timely manner. Maintains a clean and orderly work environment. Shares responsibility for ordering and stocking medication and keeping the department clean. Utilizes personal protective equipment to protect from exposure to chemical, radiation and infectious hazards.</li></ul></li><li>Professional:<ul><li>Participates in department management by providing input at meetings and reading communication. Mentors new and less experienced staff and students. Adjusts work schedule to departmental needs. Uses policy and procedure manuals and other reference material to ensure proper course of action.</li></ul></li><li>Quality Improvement:<ul><li>Identifies, proposes solutions and assists in implementation of ideas for improvement in patient care and departmental / hospital operations. Participates in the successful implementation of change. Identifies opportunities and implements strategies to decrease waste in medication. Participates in adverse drug reaction reporting and drug utilization review.
