The Physical Therapist (PT) provides evaluation and therapy services, including direct patient care, patient/family education and transition of care, in an efficient, age-appropriate, and culturally sensitive manner upon licensed independent practitioner referral. Makes appropriate discharge plans by collaborating with patients, families and the interdisciplinary team. Functions as a team member and develops leadership skills through activities such as mentoring and providing education to students and other professionals. Supports risk management and compliance initiatives through accurate, thorough documentation. This is not an all-inclusive list of this job’s responsibilities. The incumbent may be required to perform other related duties and participate in special projects as assigned. Qualifications Required: Education necessary to obtain License in State of Practice Massachusetts PT license Preferred: 1 year physical therapy experience; will consider new grads Pediatric experience
