Performance of the typical responsibilities and duties requires the ability to perform the job duties, explain to/instruct others, persuade others, involving job-related matters generally of moderate to high level of complexity.&nbsp;</p><p><u>TYPICAL RESPONSIBILITIES AND DUTIES (*: The non-essential functions)</u></p><ul><li>Targeted case management that facilitates assessment of care planning and coordination of service to approximately 15 children and families.</li><li>Ensure that medically necessary services are accessed, coordinated, and delivered in a strength-based, individualized, youth/family driven and culturally and linguistically relevant manner and that services and supports are guided by the needs of the child/ youth.&nbsp;</li><li>Facilitate a collaborative relationship among the child/ youth, his/her family and involved child-serving systems.&nbsp;</li><li>Support the parent/ caregiver in meeting their child/ youth&rsquo;s needs.</li><li>Help establish the Child and Family Team (CFT) and provide ongoing support.</li><li>Organize and match care across providers and child serving systems to allow the child/youth to be served in his/ her home community.&nbsp;</li><li>Participates in the gathering of information necessary for the creation of Coordinated Care Documents (such as CANS, SCC MH Assessment and Treatment Plan, outcome measures, etc.).</li><li>Provides quality, culturally relevant services to children and their families in community-based settings, and in-home services.</li><li>Coordinates the Child and Family Teams&rsquo; work with the youth and their supports to identify their strengths, needs and resources to meet those needs.</li><li>Coordinate with system partners in the development and periodic revision of the Treatment Plan.&nbsp;</li><li>Monitor progress to determine effectiveness of interventions and determine the need for supplementary or supportive services such as the IHBS Provider, Psychiatric Evaluation/ Medication, Individual/ Family Therapy, etc. Modifies the level of service provision to meet youth/ family needs.]</li><li>Upon achieving goals of youth/ family&rsquo;s plan, develops a transition plan to foster long term stability including the effective use of natural supports and community resources.&nbsp;</li><li>Works collaboratively with other agency staff and with individuals and agencies in the community to ensure relevance, accessibility, and quality of service provided to children and their families.&nbsp;</li><li>Documents necessary information as required within the program in a timely manner. This includes youth documentation, report writing and accurate data collection as necessary to assess progress and outcome.&nbsp;</li><li>Maintains professional standards regarding client rights.&nbsp;</li><li>Demonstrates an understanding of confidentiality policies, mandated child abuse reporting and duty to warn procedures. Utilizes clinical supervision in any case where these issues are in question.</li><li>Utilizes supervision, consultation, and training as necessary to continue to provide high quality services to children and their families as effectively and efficiently as possible.</li><li>Actively participates in provision of 24/7 crisis response.&nbsp;</li><li>Every employee has the opportunity and responsibility to participate in one or more activities each year that support the performance quality improvement (PQI) of RCS programs and services. Activities might involve membership on a PQI committee or work group, service as a peer reviewer, data collection for PQI and evaluation purposes, completion of PQI questionnaires and surveys or participation in focus groups, or other activities as identified by PQI committees or work groups.</li></ul><p>&nbsp;</p><p>MINIMUM QUALIFICATIONS</p><p><u>Education, Training, and Experience</u></p><ul><li>AMFT, ASW, APCC, PsyD/PhD or equivalent required.&nbsp;</li><li>License preferred (LCSW, LMFT, LPCC, Licensed Psychologist). If not licensed, then must be registered with the Board of Behavioral Sciences or Board of Psychology and be Licensed Waivered.&nbsp;</li><li>One year experience working with high-needs children and families. Experience working in children&rsquo;s mental health setting emphasizing a family systems perspective is preferred.</li><li>Advanced skills in crisis intervention, suicide assessment, conflict resolution, and interpersonal communication are preferred.</li><li>Bilingual/ Bicultural (Spanish/English) is strongly preferred.</li><li>Experience working with Department of Social Services, Mental Health, and other larger county systems.&nbsp;</li></ul><p><u>License(s)</u></p><ul><li>California driver&rsquo;s license (and good DMV report), ability to be insured by agency insurance carrier.&nbsp;</li></ul><p><u>Knowledge and Skills</u></p><ul><li>Leadership and organizational skills.</li><li>Program development skills and case facilitation knowledge.</li><li>Strong ability to work as part of a team.</li><li>Ability to complete Assessments and develop and implement individualized Treatment Plans.</li><li>Excellent time management skills.</li><li>Mental Health Rehabilitation Option knowledge.</li><li>Foster care system knowledge.</li><li>Knowledge of and ability to access other community resources.</li><li>Ability to work with a balance of independence and creative use of supervision from the Clinical Program Manager.</li><li>Ability to think creatively and intervene/respond in crisis situations.</li><li>Ability to coordinate and manage volunteers, services, and staff.&nbsp;</li></ul><p><u>Special Requirements</u></p><ul><li style=\"font-size: 10px;\"><h1><span style=\"font-size: 12px;\">Must be able to meet and receive a criminal records clearance as required by Title XXII licensing regulations.</span></h1>&nbsp;</li></ul><p><u>Physical and Mental Requirements</u></p><p>The following are required in day-to-day performance of the duties of this position:&nbsp;</p><ul><li>Sitting, walking, standing, grasping (simple), and visual/auditory acuity are constantly required (over 2/3 of the workday).</li><li>Bending, squatting, kneeling, twisting, lifting, reaching, and pushing/pulling are required occasionally (under 1/3 of the workday).</li><li>Climbing and crawling are generally not required.</li></ul></div></div></div></div></div>\n",
