    "description": "<h2>Min</h2>USD $95,000.00/Yr.\n<h2>Max</h2>USD $100,000.00/Yr.\n<h2>Position Overview</h2>\n<p><strong>SCOPE OF ROLE:</strong>The Registered Nurse will work on OMH care team/s with mental health professionals assisting clients in transition from hospitals back to the community. The team will follow individuals into the community using the evidence-based Critical Time Intervention model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The Registered Nurse is responsible for the diagnosis and treatment of human responses to actual or potential health problems through such services as medical case management, referrals, health teaching (this might include group education) and counseling. The nurse will provide interventions supportive to or restorative of life and well-being; and execute medical regimens prescribed by a physician, PNP or another licensed health care provider. The role will require limited on call coverage.</p>\n<p> </p>\n<p><strong>ESSENTIAL DUTIES & RESPONSIBILITIES:</strong></p>\n<p> </p>\n<ul>\n <li>Complete medical assessments and Communicable Disease Risk Assessments within mandated time frames</li>\n <li>Coordinate delivery and reception of medications with pharmacy</li>\n <li>Track all medications received and administered as well as coordinate return of unused medications to pharmacy</li>\n <li>Observe self-administration of medications for detoxification according to established protocols</li>\n <li>Refer for required medical services</li>\n <li>Carry out urine screenings in accordance with program policy</li>\n <li>Provide education and consultation to individual clients and/or groups on their health needs</li>\n <li>Assess clients’ medical record and coordinate medical care with medical director and/or outside clinics</li>\n <li>Assure clients’ compliance with prescribe medical regimen</li>\n <li>Complete timely and accurate documentation on medical services rendered</li>\n <li>Keep supervisor informed of all relevant information regarding the residents and the program</li>\n <li>Other duties as requested by supervisor</li>\n</ul>\n<p> </p>\n<p><strong>Schedule: Monday - Friday 9 am - 5p</strong></p>\n<p>  </p>\n<p><strong>Benefits Overview: </strong>We offer attractive compensation with comprehensive benefits including: Medical/Dental/Prescription/Vision/Life Insurance; 403(b); Credit Union; FSAs; Short-and-Long-Term Disability; Transportation Plan; Generous Paid Vacations and Holidays</p>\n<p> </p>\n<p><strong>IND1 #CB</strong></p>\n<h2>Qualifications</h2>\n<p><strong>REQUIRED EDUCATION AND EXPERIENCE </strong></p>\n<ul>\n <li>NYS RN license</li>\n <li>Knowledge of chemical dependency as a primary diagnosis.</li>\n <li>Excellent written and verbal communication</li>\n <li>Assessment and case management</li>\n <li>Knowledge of Microsoft Office tools</li>\n <li>Ability to provide crisis intervention utilizing good interpersonal skills with a high level of maturity and responsibility </li>\n</ul>\n<p><strong>PREFERRED QUALIFICATIONS & SKILLS</strong></p>\n<ul>\n <li>Basic knowledge of human services</li>\n <li>Bilingual (Spanish/English) speaking preferred</li>\n</ul>\n<p> </p>\n<p> </p>\n<h2>Company Overview</h2>\n<p><strong>S:US IS AN EQUAL OPPORTUNITY EMPLOYER</strong> </p>\n<p> </p>\n<p>Join a team of employees who care about the wellbeing of others.
