Uses time spent in the mobile MAT unit/county jails to assist in initiating MAT services by providing evaluation, screening and assessment services and to engage patients in on-going treatment and recovery support. Assists in post-incarceration handoff to traditional outpatient MAT for continued treatment and develops a transition plan to support care coordination and referrals with other community providers with recovery support services (such as peer recovery coaching, employment assistance, childcare, counseling, overdose prevention classes, and housing support) to increase retainment in treatment, support long-term recovery, and reduce recidivism.
