Description
The Admissions & Referral Intake Specialist supports admissions to the facility, in-house transfers and discharge processing for the Skilled Nursing, TCU and Assisted Living programs. Works in a team-based setting to work collaboratively with hospitals, skilled nursing facilities, referral agencies, discharge planners, social workers, and in-house staff to achieve budgeted census levels.
Assists the Mount Leadership Team in outreach activities to enhance the ministry visibility with the greater community.

Required qualifications:
2 years of experience in long term care, clinical case management and admissions. 2 years of experience in a customer service, business development or sales environment. Preferred qualifications:
