This position also is responsible for connecting\nhigh-risk individuals and facilitate access to\ncommunity service providers (CSPs) that can meet their health related social\nneeds. This position actively participates in special activities of the\nDepartment. It is expected to work within the program directives of the funding\nsource, within the philosophical framework set by the Board of Directors, and\nwithin the existing network of community services providers and natural\nhelpers.</p><p style=\"margin: 1em 0px;\"><b style=\"mso-bidi-font-weight: normal;\">EDUCATION/EXPERIENCE:</b></p><p>1. High School Diploma or equivalent required</p><p>2. Community Health Worker Certification or college degree preferred</p><p>3. Three years of experience in a health care setting preferred</p><p>4. Experience and competency in Public Benefit Applications and community outreach work preferred&nbsp;</p><p>5. First Aid/CPR Certified</p><p>6. A current Hawaii State Driver&rsquo;s License, daily access to an automobile with current registration, safety check and the minimum liability insurance as required by the &ldquo;Hawaii No-Fault Law&rdquo; required</p><p style=\"margin: 1em 0px;\"><br></p><p style=\"margin: 1em 0px;\"><br></p><p style=\"margin: 1em 0px; text-align: center;\"><strong id=\"isPasted\"><em>An Equal Employment Opportunity / Affirmative Action Employer</em></strong></p></div></div></div></div></div>\n",
