    "ExternalDescriptionStr": "<p>We are searching for a Care Coordinator Assistant. &nbsp;Someone who will provide non-clinical administrative support to the Care Coordination process including but not restricted to timely discharge planning and utilization management.</p><p>&nbsp;</p><p>Think you've got what it takes?</p><p>Qualifications:</p><ul><li>H.S. Diploma or GED required</li><li>Some college coursework preferred</li><li>2 years' experience in hospital care coordination/case management, home health, quality improvement or a related field required</li></ul><p>Responsibilities:</p><ul><li>Provides department-related and other appropriate communications to internal and external customers.</li><li>Provides assistance for utilization management processes to maintain compliance with federal, state and managed care guidelines.</li><li>Provides assistance for transition/discharge planning and care coordination processes to transition plans and enhance patient/family satisfaction.</li><li>Maintains data integrity.</li><li>Provides professional, efficient and courteous service to the customer while maintaining integrity in dealing with issues of a confidential nature.&nbsp;</li><li>Works effectively with peers, nursing and medical staff and maintains a positive attitude. Responds to requests personally, tactfully, and courteously.&nbsp;</li><li>Obtains information required for response whenever possible and communicates pertinent information to staff, customers, and providers. Relays messages to appropriate individuals efficiently and in a timely manner.&nbsp;</li><li>Promotes a positive work environment, and contributes to the overall team effort.</li></ul>",
