This position partners closely with residents, families, clinical teams, and external agencies to ensure smooth transitions, emotional support, and access to vital resources.</p><p>You’ll be a trusted advocate, a collaborative teammate, and a calm presence during some of life’s most challenging moments.</p><p><strong><u>What You’ll Do</u></strong></p><p><strong>Resident Advocacy &amp; Social Services</strong></p><ul><li>Assess resident psychosocial, emotional, and medically related social needs.</li><li>Provide direct social services or connect residents with appropriate community resources.</li><li>Assist residents with financial assistance applications and outside agency referrals.</li><li>Maintain accurate, confidential social service documentation and progress notes.</li><li>Educate residents and families on resident rights.</li></ul><p><strong>Care Planning &amp; Assessments</strong></p><ul><li>Complete initial social service assessments for all admissions.</li><li>Maintain MDS social service sections and conduct quarterly assessments for LTC residents.</li><li>Participate in and coordinate interdisciplinary care plan meetings.</li></ul><p><strong>Discharge Planning &amp; Transitions</strong></p><ul><li>Serve as the facility’s Discharge Planning Coordinator.</li><li>Coordinate admissions, transfers, and discharges with internal teams and external partners.</li><li>Communicate discharge and transfer reports.</li><li>Track Health Center days for independent living residents.</li></ul><p><strong>Admissions &amp; Community Engagement</strong></p><ul><li>Partner with the Admissions Coordinator to provide tours and admission information.</li><li>Serve as backup for admissions processes, including interviews, documentation, and financial data collection.</li><li>Collaborate with Finance, Marketing, and other interdisciplinary teams.</li><li>Attend weekly Levels of Care meetings.</li><li>If applicable, assist with clinical liaison marketing efforts and discharge follow-ups.</li></ul><p><strong><u>Qualifications We’re Looking For</u></strong></p><ul><li>Bachelor’s degree in Social Work, Gerontology, or a related field.</li><li>Current Social Worker license in the state of CT.&nbsp;</li><li>Minimum of 2 years’ experience in senior services or a related field.</li><li>Strong communication, documentation, and interpersonal skills.</li><li><p>Professional presence and the ability to navigate emotionally complex situations with grace.</p><p>&nbsp;</p></li></ul><p><i>If you’re passionate about HR and motivated by a culture that values people first, we’d love to hear from you!
