The OT utilizes <strong>GG (Functional Independence Measure &ndash; FIM/GG) scoring</strong> to assess patient progress and document functional outcomes in compliance with CMS (Centers for Medicare &amp; Medicaid Services) and other regulatory requirements.</p><p><strong>POSITION&rsquo;S ESSENTIAL RESPONSIBILITIES:&nbsp;</strong></p><p><strong>1. Patient Evaluation &amp; Assessment (Acute &amp; IPR):</strong></p><ul><li>Conduct <strong>bedside evaluations</strong> in <strong>acute care</strong> for medically fragile patients (ICU, step-down, medical/surgical units).</li><li>Perform <strong>comprehensive rehab assessments</strong> in <strong>IPR</strong>, focusing on ADLs, mobility, cognition, and discharge readiness.</li><li>Administer <strong>GG scoring (Section GG)</strong> for IPR patients per CMS guidelines (e.g., GG0130: Eating, GG0170: Sitting to Standing).</li><li>Use <strong>standardized assessments</strong> (e.g., MoCA, MMT, FIM) in both settings.</li></ul><p><strong>2. Treatment Planning &amp; Implementation:</strong></p><p><strong>Acute Care Focus:</strong></p><ul><li><strong>Early mobilization</strong> of ICU/post-surgical patients to prevent deconditioning.</li><li><strong>Energy conservation &amp; compensatory strategies</strong> for patients with cardiac/pulmonary conditions.</li><li><strong>Dysphagia &amp; adaptive feeding techniques</strong> (collaborating with SLPs).</li><li><strong>Splinting &amp; edema management</strong> for post-op orthopedic/neuro patients.</li></ul><p><strong>IPR Focus:</strong></p><ul><li><strong>High-intensity ADL retraining</strong> (dressing, bathing, toileting).</li><li><strong>Neuromuscular re-education</strong> for stroke/TBI recovery.</li><li><strong>Cognitive-perceptual rehab</strong> (memory, attention, executive function).</li><li><strong>Home assessments &amp; caregiver training</strong> pre-discharge.</li></ul><p><strong>3. GG Scoring &amp; Documentation (IPR) + Acute Care Requirements:</strong></p><ul><li><strong>For IPR:</strong> Accurately complete <strong>Section GG functional scoring</strong> at admission, discharge, and per IRF-PAI guidelines.</li><li><strong>For Acute Care:</strong> Document <strong>medically necessary skilled OT services</strong> (e.g., progress notes, DC planning).</li><li>Ensure compliance with:<ol><li><strong>CMS guidelines</strong> (Medicare Part A, IRF coverage criteria).</li><li><strong>Hospital policies</strong> (infection control, fall prevention).</li><li><strong>Joint Commission standards</strong> (safety, quality measures).</li></ol></li></ul><p><strong>4. Discharge Planning &amp; Interdisciplinary Coordination:</strong></p><ol><li><strong>Acute Care:</strong> Recommend DME, home health, or SNF placement based on patient stability.</li><li><strong>IPR:</strong> Coordinate with case managers for <strong>safe community reintegration</strong>.</li><li>Educate patients/families on <strong>post-discharge care</strong> in both settings.</li></ol><p><strong>5.
