Description

Certified (PSS/PRC) for Mental Health Treatment Court : OnPoint is seeking a Certified Peer Support Specialist and Recovery Coach for Mental Health Treatment Court.  In this role you will s upport individuals in their unique wellness and recovery journey through mentorship, coaching and empathy, including skill building, problem-solving and appropriately sharing one’s own recovery journey. PAY RANGE/BENEFIT PACKAGE: Salary: starting at $43,243.20 - $59,675.20 annually - placement above minimum salary is based on experience. Primary diagnosis of a mental health condition. Have a strong personal knowledge of what it is like to have first-hand lived experience with a mental health condition that has caused a substantial life disruption. Has been a recipient of public or private mental health treatment and/or services for at least one year, with a substantial life disruption due to their mental health condition. Have personal experience in navigating complex mental health treatment services. Self-identifies as having a mental health condition with a substantial life disruption and shares their recovery story in supporting others. State of Michigan Certified Peer Recovery Coach or be eligible to become certified by the State as a Peer Recovery Coach, which requires an individual to have: At least 2 years of long-term, stable recovery from alcohol and other drugs. Able to identify with another individual based on their shared life experiences to promote recovery by removing barriers and obstacles to recovery. Has been a recipient of substance abuse services for at least one year, with a substantial life disruption due to their substance abuse condition. Have personal experience with navigating complex substance abuse treatment services. At least 3 years behavioral health experience or other equivalent verifiable experience PREFERRED QUALIFICATIONS: Experience with the Michigan Court Systems Care coordination experience Previous program development or leadership experience Professional experience as a peer support specialist or peer recovery coach GENERAL RESPONSIBILITIES: In accordance with funding, regulatory and best practice standards, deliver crisis peer services as outlined in each client’s individual plan of service. Provide services to Mental Health Treatment Court participants by meeting with them, helping to secure housing, assisting with employment supports, and accommodating other transportation needs as approved for each program participant by the mental health treatment court. Attend Mental Health Treatment Court meetings and sessions bi-weekly. Document in the court EMR and well as organization’s EMR Build and maintain relationships with and coordinate with Allegan District Court, Prosecutor’s office and Public Defender Offices Coordinate treatment between internal treatment teams and Mental Health Treatment Court clients and responsibilities. Provide care coordination and case management for assigned individuals under the direction of the court program or assigned clinician including: Internal and external case collaboration and coordination, including gathering and exchanging relevant information. Development of Natural Supports including family, friends, religious organizations/leaders, neighbors, teachers, employers, and all others involved and interested in supporting the consumer. Participation in clinical case conferences, staff development (in- services, conferences, professional organizations) and agency committees as assigned. Support individuals on assigned caseload through advocacy, monitoring, referring, and linking with other providers. Facilitate wellness, recovery, and other group interventions. Facilitate symptom-management techniques and promote personal growth and development by assisting clients in coping with external and internal stressors following an access intake or crisis intervention. Provide individuals with direct support navigating the healthcare system, accessing resources, supporting a person-centered recovery journey to achieve community inclusion, independence and resiliency. Provide 24-hour follow-up calls to individuals in crisis leaving higher levels of care. Participate with and/or assist in engaging in community education and wellness and recovery support opportunities. Participate in supervision, team and staff meetings, treatment planning, quality improvement, and continuing education.  Seek support and direction from clinicians when situations exceed the scope of practice. Document services according to funding, regulatory and best practice standards. Perform other duties as assigned. EQUIPMENT/TECHNOLOGY KNOWLEDGE:  Efficient in Microsoft Office 365 Efficient in Electronic Medical Record system (EMR) POSITIONS TO BE FILLED:  One full-time position (40 hours/week)
