    "description": "<p><span style=\"font-family: arial, helvetica, sans-serif; font-weight: bold\">POSITION SUMMARY:</span><br><span style=\"font-family: arial, helvetica, sans-serif\">The clinic nurse/referral nurse works as part of the clinic provider team and assists the providers in the</span><br><span style=\"font-family: arial, helvetica, sans-serif\">provision of primary health care. The nurse provides limited direct patient services and performs</span><br><span style=\"font-family: arial, helvetica, sans-serif\">specifically designated independent procedures. The clinic nurse is also a member of the health care team</span><br><span style=\"font-family: arial, helvetica, sans-serif\">and therefore, takes part in the delivery of care in accordance with the Patient Centered Medical Home</span><br><span style=\"font-family: arial, helvetica, sans-serif\">model of care. The nurse is responsible for ensuring that the transition of care across the health care</span><br><span style=\"font-family: arial, helvetica, sans-serif\">spectrum is smooth and coordinated.</span></p>\n<p><br><span style=\"font-family: arial, helvetica, sans-serif; font-weight: bold\">RESPONSIBILITIES (Include but not limited to):</span></p>\n<ul>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Prepares patients for examination. </span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Collects labs and administers injections as ordered by clinician.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Be the systems navigator and point of contact for patients and families, with patients and families</span><br><span style=\"font-family: arial, helvetica, sans-serif\">having direct access for asking questions and raising concerns. May assume advocate role on the</span><br><span style=\"font-family: arial, helvetica, sans-serif\">patient’s behalf with the carrier to ensure approval of the necessary supplies/services for the</span><br><span style=\"font-family: arial, helvetica, sans-serif\">patient in a timely fashion.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Perform accurate medication reconciliation to ensure patient safety and continuity of care.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Review and update patient problem lists in the electronic medical record to maintain </span><br><span style=\"font-family: arial, helvetica, sans-serif\">current and comprehensive documentation.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Make appointments as needed.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Assemble information concerning patient’s clinical background and referral needs. Per referral </span><br><span style=\"font-family: arial, helvetica, sans-serif\">guidelines, provide appropriate clinical information to specialist.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Triage’s patient calls, asking for medical advice from clinician to answer patient questions.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Refills medications per policy. </span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Identify and utilize cultural and community resources.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Assists providers in healthcare activities, performance of diagnostic and/or therapeutic</span><br><span style=\"font-family: arial, helvetica, sans-serif\">procedures, and the administration of medications as necessary.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Assists with stocking the examining room and special treatment rooms with needed supplies..</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Performs other necessary duties as required by the Clinic Director or Nursing Director to meet the</span><br><span style=\"font-family: arial, helvetica, sans-serif\">goal of providing primary health care services.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Conducts patient screenings and routine visits.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Collects specimens.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Performs routine laboratory procedures. </span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Provide patient-centered care that incorporates culturally and linguistically appropriated services.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Educate patients about health maintenance activities, adoption of healthy behaviors, and support</span><br><span style=\"font-family: arial, helvetica, sans-serif\">services available in the community.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Communicate with patient and families about health care issues and the importance of</span><br><span style=\"font-family: arial, helvetica, sans-serif\">developing self-management goals and healthy behaviors.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Assist in coordinating seamless transitions of patient care across multiple care settings by using</span><br><span style=\"font-family: arial, helvetica, sans-serif\">combined expertise of care team members.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Employ active listening techniques during interactions with patients, families, and team members.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Utilize quality data to identify process changes that will improve care at the individual patient and</span><br><span style=\"font-family: arial, helvetica, sans-serif\">population management.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Use health information systems efficiently to manage and improve care at the individual patient</span><br><span style=\"font-family: arial, helvetica, sans-serif\">level and system levels and ensure the delivery of timely and accurate information..</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Participate in care team huddles and/or pre-visit planning to prepare and coordinate patient care</span><br><span style=\"font-family: arial, helvetica, sans-serif\">activities.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Incorporate quality indicators for specific disease states and /or patient variable into patient care</span><br><span style=\"font-family: arial, helvetica, sans-serif\">plans.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Participates in the community health center's quality improvement activities and performs duties</span><br><span style=\"font-family: arial, helvetica, sans-serif\">in accordance with applicable standards.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Ensures an appropriate environment for the administration of healthcare by keeping exam rooms,</span><br><span style=\"font-family: arial, helvetica, sans-serif\">nurses’ station, all equipment, and other work areas in a clean, safe, and orderly fashion.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Attend in house PCMH training.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">Other duties assigned. </span></li>\n</ul>\n<p><br><span style=\"font-family: arial, helvetica, sans-serif;
