Together, our interrelated programs provide a whole-person, whole-family, and whole-life approach that unlocks solutions for a lifetime.</p>\n<p> </p>\n<p>Interventionist will provide direct home-based interventions to address risk factors, increase protective factors, and ensure the safety of families. </p>\n<h2>Responsibilities</h2>\n<ul>\n <li><p>Reports to assigned program supervisor or covering supervisor. </p></li>\n <li><p>Provides home-based clinical treatment utilizing the Functional Family Therapy – Therapeutic Case Management (FFT-TCM) model to assigned caseload of up to 10 cases and provides coverage for peer's caseload as needed  </p></li>\n <li><p>Manages the extensive travel requirements involved in serving families in their homes. </p></li>\n <li><p>Conducts assessments including intakes, review of referral information, identifying and engaging key participants, identifying relational patterns, and individualized treatment planning. </p></li>\n <li><p>Engages families to sign for services and completes intakes with families within the required ACS timeframes. </p></li>\n <li><p>Participates in all required agency, model and stakeholder trainings, individual and group coaching and staff meetings. </p></li>\n <li><p>Adheres to model expectations and implement with fidelity including the timely completion of progress notes, session planning guides, session impact scales and appointment notes in the Care4 documentation system.  </p></li>\n <li><p>Actively participate in weekly consultation and supervision and site visits when they occur. </p></li>\n <li><p>Consistently meets all program and model Key Performance Indicators (including session pacing, required casework contacts, successful client outcomes, timely completion of Family Assessment Service Plans, data entry in all systems, etc.). </p></li>\n <li><p>Identify and address family risk factors that give rise to possible child removal, ACS involvement, and out of home placement due to continued referral behavior or new risk/safety factors. This includes the completion of safety plans and incident reports if necessary. </p></li>\n <li><p>Maintains clear, concise and timely documentation of treatment efforts in progress notes and Family Assessment and Service Planning (FASP) that demonstrate compliance with the FFT-TCM Model and supervisory review feedback.  </p></li>\n <li><p>Manages the extensive documentation requirements in all databases to successfully comply with mandates from agency, ACS and model. </p></li>\n <li><p>Identify community-based resources for families, collaborate and build relationships with stakeholders/providers (i.e., ACS, schools, doctors, mental health providers, courts, etc.) to monitor participation in services and accompany families to appointments if needed. </p></li>\n <li><p>Provides 24/7 coverage during the week and participates on a weekend on-call rotational schedule as applicable.  </p></li>\n <li><p>Other duties as assigned. </p></li>\n</ul>\n<p><strong>Organizational expectations include: </strong></p>\n<ul>\n <li><p>Regular and punctual time and attendance. </p></li>\n <li><p>Consistent demonstrated commitment to The Foundling’s BHAG, mission, vision and values (Compassion, Dignity, Family and Community, Diversity, Excellence). </p></li>\n <li><p>Adherence to The Foundling’s employee policies, Code of Conduct and applicable rules, laws, regulations or bargaining agreements.  </p></li>\n</ul>\n<p> </p>\n<p><strong>The New York Foundling is committed to attracting and retaining a diverse employee population, the Foundling will honor your experiences, perspectives and unique identity.
