Through a variety of multidisciplinary services that clients can choose from including medication management, therapy, group therapy, health and wellness support, employment services and transportation support, clients can develop skills, access resources and learn ways to manage their emotional and physical health.&nbsp;</p>\n\n<p>&nbsp;</p>\n\n<p><strong>At ReDiscover, while every day is different, a typical day may include the following roles and responsibilities:</strong></p>\n\n<ul>\n\t<li>Maintain and interact with a caseload of clients with diverse characteristics&nbsp;either in-person or over the phone.</li>\n\t<li>Facilitate each client&rsquo;s achievement of long range outcomes of symptom&nbsp;management, employment and independent living based upon the needs of the&nbsp;client.</li>\n\t<li>Engage difficult to serve clients by investigating all potential resources in order&nbsp;to gain access and make connections.</li>\n\t<li>Help remove barriers for clients, which may include helping clients participate&nbsp;in community or social service programs and referring clients to educational or&nbsp;working programs.</li>\n\t<li>Assist&nbsp;clients with&nbsp;transportation barriers as identified in&nbsp;their treatment plan.&nbsp; This includes transporting clients in the employee&#39;s personal vehicle to various appointments and area resources.</li>\n\t<li>Collect and maintain information about clients and document interactions&nbsp;within ReDiscover&rsquo;s designated electronic medical record.</li>\n\t<li>Communicate and collaborate with an internal multi-disciplinary team to&nbsp;address various aspects of wellness and ensure positive communication&nbsp;regarding the coordination of client care.</li>\n</ul>\n",
