The Speech-Language Pathologist assists in ensuring the health and well-being of residents by developing and implementing individualized speech-language therapy treatment plans and providing high-quality therapeutic services.</p>\n<p>This position plays a vital role in maintaining the highest degree of quality care at all times.</p>\n<h3>Responsibilities</h3>\n<ul>\n <li><p>Evaluate residents to determine speech, language, cognitive-communication, and swallowing needs</p></li>\n <li><p>Develop and implement individualized treatment plans based on evaluation findings</p></li>\n <li><p>Provide speech-language therapy services in accordance with physician orders and facility standards</p></li>\n <li><p>Conduct audiometric screenings upon request and recommend further audiology services as appropriate</p></li>\n <li><p>Document evaluations, treatment plans, progress notes, and discharge summaries in compliance with regulatory standards</p></li>\n <li><p>Collaborate with interdisciplinary team members to ensure coordinated resident care</p></li>\n <li><p>Ensure compliance with federal, state, and facility regulations</p></li>\n</ul>\n<h3>Qualifications</h3>\n<ul>\n <li><p>Master’s degree in Speech-Language Pathology</p></li>\n <li><p>Current state licensure as a Speech-Language Pathologist (Vermont)</p></li>\n <li><p>Certificate of Clinical Competence (CCC-SLP) preferred</p></li>\n <li><p>Experience in skilled nursing, rehabilitation, or long-term care preferred</p></li>\n <li><p>Strong communication and documentation skills</p></li>\n</ul>\n<h3>Why Join Elderwood at Burlington?</h3>\n<ul>\n <li><p>Competitive PRN rate of <strong>$65/hour</strong></p></li>\n <li><p>Flexible scheduling</p></li>\n <li><p>Supportive interdisciplinary team</p></li>\n <li><p>Opportunity to make a meaningful impact in the lives of residents</p></li>\n</ul>\n<h2>Responsibilities</h2>\n<p><strong>Speech-Language Pathologist:</strong></p>\n<ul>\n <li>Screens resident for communication problem upon request; conducts an in-depth evaluation and makes written recommendations for a therapy plan of care, if necessary and upon request.</li>\n <li>Coordinates audiometric screening test for resident and makes referrals for further audiology evaluations, or treatments if necessary.</li>\n <li>Initiates speech-language therapy plan of care according to physician orders; and maintains required contact with the physician during the therapy program of resident.</li>\n <li>Observes rights of residents and treats residents with respect and kindness.</li>\n <li>Maintains written evaluations, progress notes, and discharge summaries for all residents in a speech-language treatment program.</li>\n <li>Confers with other staff regarding special needs of residents and integrates speech-language treatment program with goals and approaches of other disciplines.</li>\n <li>Attends Resident Care Planning and Family/Resident Care Plan Review meetings for residents currently in a speech-language treatment program.</li>\n <li>Maintains records that include statistical reports about resident, in-service education information, budget information and a material/equipment inventory.</li>\n <li>Submits statistical reports to the Business Office staff for Medicare billing payments, and completes assessment forms upon request.</li>\n <li>Attend staff meetings and required or preferred in-service training.</li>\n <li>Participates in developing and conducting in-service training programs for facility personnel involved in resident care.</li>\n <li>Reviews written speech-language policies and procedures annually and makes necessary revisions.</li>\n <li>Maintains costs of program within the parameters of the annual budget.</li>\n</ul>\n<h2>From</h2>\n<h2>Up to</h2>\n<h2>Qualifications</h2>\n<p><strong>Speech-Language Pathologist:</strong></p>\n<ul>\n <li>M.A. or M.S. Degree in Speech-Language Pathology from an approved school required.</li>\n <li>State license to practice as a Speech-Language Pathologist required.</li>\n <li>Clinical Competence Certification from the American Speech and Hearing Association preferred.</li>\n <li>Ability to communicate well verbally and in writing required.</li>\n <li>Current, valid CPR/BLS credential.</li>\n <li>Qualities of maturity, diplomacy, and ability to work well with others required.</li>\n <li>Verification of qualifications by the State Department of Health</li>\n <li>Rehabilitation Therapies (license number/certification number) required.</li>\n <li>Ability to perform tasks to established standards of excellence required.</li>\n <li>Demonstrated willingness to uphold the vision and mission of the company by following the core values of Integrity, Team work, Passion for Excellence and Reverence for the Individual is required.</li>\n <li>This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
