Our navigation team helps individuals and families access essential resources and services to meet their personal, financial, educational, and social needs.</p>\r\n\r\n<p>The Community Health Navigator serves as a frontline public health worker and acts as a liaison between AccessHealth clinics and community resources. This role is responsible for connecting patients and community members with healthcare and social services to improve overall well-being.</p>\r\n\r\n<hr />\r\n<h2><strong>Essential Job Duties and Responsibilities</strong></h2>\r\n\r\n<ul>\r\n\t<li>Represent AccessHealth at community meetings, events, and collaborative partnerships.</li>\r\n\t<li>Assess patients&rsquo; social determinants of health and identify needs.</li>\r\n\t<li>Provide health coaching and education to patients and community members.</li>\r\n\t<li>Assist individuals in accessing resources such as food, housing, health insurance, transportation, and healthcare services.</li>\r\n\t<li>Follow up with assigned patients to ensure successful access to care and services.</li>\r\n\t<li>Coordinate referrals and connect clients with appropriate external agencies.</li>\r\n\t<li>Participate in community outreach events, including health fairs and presentations.</li>\r\n\t<li>Accurately document all activities and services in patient records in a timely manner.</li>\r\n\t<li>Maintain ongoing communication with patients between visits.</li>\r\n\t<li>Serve as a liaison between the clinic and community organizations.</li>\r\n\t<li>Participate in ongoing training and professional development opportunities.</li>\r\n\t<li>Ensure compliance with organizational policies, procedures, and regulatory requirements.</li>\r\n\t<li>Perform additional administrative or related duties as assigned.</li>\r\n</ul>\r\n",
