    "description": "<h2>Salary</h2>The salary range for this position is CAD $32.84/Hr. - CAD $41.35/Hr.\n<h2>Job Accuracy Posting Flag</h2>The hours of work, including start and stop times, days off, work area and work site may be subject to change consistent with operations requirements and applicable provisions of the Collective Agreement and statutes. **Testing may be required. ***A criminal record check may be required\n<h2><p>Summary</p></h2>\n<p>Reporting to the Patient Care Manager or Resident Care Manager and working in collaboration with the interdisciplinary team, the Licensed Practical Nurse (LPN) is responsible for providing nursing care to an assigned group of patients/residents according to established policies and procedures and the Standards of Practice for the Licensed Practical Nurses of British Columbia. The LPN functions as an independent practitioner, while recognizing situations such as increased patient acuity or care that falls outside the scope of an LPN as appropriate for accessing the RN/RPN for collaborative support and/or clinical intervention.</p>\n<ul>\n <li>Assists patients/residents in meeting physical, social, spiritual, and psychological needs</li>\n <li>Consistent with mission, vision, and values of Providence Health Care (PHC)</li>\n <li>Implements established protocols</li>\n <li>Participates in care planning, assessments, interventions, and evaluations</li>\n <li>Documents information and advocates for patients/residents</li>\n <li>Attends various meetings including care conferences</li>\n <li>Assists with orientation of new staff and students</li>\n <li>Responds to and reports incidents following established procedures</li>\n <li>Performs clerical duties as required</li>\n</ul>\n<h2>Minimum Nurse-to-Patient Ratio</h2>Minimum Nurse-to-Patient Ratios (mNPR) set the minimum number of nurses required to safely care for patients on a given unit. The new St. Paul’s Hospital is expected to open in 2027.</li>\n <li><strong>Program and Scope: </strong>Holy Family Hospital Rehabilitation Unit (HFH Rehab) is one of the largest rehabilitation referral centres in BC. The unit has 65 inpatient beds dedicated to supporting the specialized rehabilitation needs of older adults following a life-altering event, such as a stroke, other neurological issues, orthopedic trauma, as well as major surgeries like hip replacements and leg amputations. In addition, more than 15,000 patients visit HFH Rehab each year to receive outpatient rehabilitation services for needs resulting from illness or injury</li>\n</ul>\n<p><strong>Your Day to Day</strong></p>\n<ul>\n <li>Provide safe and respectful nursing care, adhering to established policies, individualized care plans, and priorities</li>\n <li>Conduct thorough assessments, monitor signs/symptoms, and communicate with the RN/RPN as needed</li>\n <li>Prepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary team</li>\n <li>Administer medications per protocol, monitor effects, and maintain communication with the team</li>\n <li>Utilize evidence-based resources to stay updated on nursing practices</li>\n <li>Respond to incidents such as codes, injuries, spills, and alarms, following procedures</li>\n <li>Assist with staff orientation, demonstrating procedures and providing support</li>\n <li>Address patient/resident and family learning needs, provide educational materials, and make referrals</li>\n <li>Attend care planning meetings, provide input, and advocate for patients/residents</li>\n <li>Collaborate with the team to develop and evaluate individualized care plans</li>\n <li>Assist with discharge planning based on patient status and external supports</li>\n <li>Operate and clean equipment, report malfunctions, and handle hazardous materials.
