    "description": "<p><br><br></p>\n<p><span>Since 1985, Albuquerque Health Care for the Homeless has dedicated its mission exclusively to providing services to people experiencing homelessness. AHCH provides a distinctive continuum of integrated care through outreach and site-based services to address the health-related causes and consequences of homelessness. Our vision is to live in a world that is just and without homelessness. We believe that homelessness is an issue that can be solved through access to quality health care, adequate and affordable housing, and a living wage.</span></p>\n<p><br></p>\n<p>Our ideal candidate is someone with strong critical thinking skills, a client-centered mindset, and the ability to work well with diverse populations. The candidate must be comfortable with traveling to clients in various parts of the city and transporting clients in our company vehicles.</p>\n<p><br></p>\n<p>AHCH practices a harm reduction approach in all services and we seek candidates that embrace and uphold this philosophy and provide services within a trauma informed framework. We are a housing first, treat first organization that strives to remove as many barriers to health care and housing as possible. </p>\n<p><br></p>\n<p>CSW caseloads can vary from 20-30 depending on client acuity and it is expected that the CSW will have a minimum of 60 encounters per month. Additionally, documentation of encounters must be completed within 72 hours of the visit date.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Duties and Responsibilities:</span></p>\n<p><br></p>\n<ul>\n<li>Assess individual needs of clients</li>\n<li>Create individualized service plans with direct input from client in accordance with CCSS<br>requirements</li>\n<li>Work in an integrated manner with other AHCH programs to optimize client care</li>\n<li>Refer client to appropriate programs and agencies for coordination of wrap-around<br>services in the community</li>\n<li>Participate in client chart audits for peer review</li>\n<li>Provide direct service to clients with 60% of contacts conducted in home<br>visits/community visits</li>\n<li>Perform duties in a trauma informed and client centered manner</li>\n<li>Ensure accurate and appropriate documentation of all client contacts in Electronic Health<br>Record (EHR) within timeframe required by AHCH procedure and billing standards</li>\n<li>Attend program-specific meetings and trainings including clinical supervision with<br>Behavioral Health Clinical Director</li>\n<li>Attend organizational meetings and trainings</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Minimum Qualifications:</span></p>\n<p><br></p>\n<ul>\n<li>Ability to work in a client centered, trauma informed, and harm reduction manner to assist clients in meeting the goals that they develop<br></li>\n<li>NM driver's license (or the ability to obtain an NM driver's license within 30 days of start date) and clean driving record</li>\n</ul>\n<p><br></p>\n<ul>\n<li>A bachelor’s degree in a human service field from an accredited university and one year relevant experience with the target population<br></li>\n</ul>\n<p><span style=\"text-decoration: underline\">OR</span></p>\n<ul>\n<li>An associate’s degree and a minimum of two years of experience working with the target population</li>\n</ul>\n<p> <span style=\"text-decoration: underline\">OR</span><br></p>\n<ul>\n<li>A high school graduation or general educational development (GED) test and a minimum of three years of experience working with the target population</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Preferred Qualifications:</span></p>\n<p><br></p>\n<ul>\n<li>CCSS certification</li>\n<li>Experience in trauma-informed care and harm reduction models</li>\n<li>Bilingual or Spanish speaking<br></li>\n</ul>\n<p><br></p>\n<p><span> All benefits start the month after you begin work:</span></p>\n<p><br></p>\n<ul>\n<li><span>Low cost medical, vision, and dental insurance with health club membership</span></li>\n<li><span>Life insurance and Accidental Death and Dismemberment fully paid for by organization</span></li>\n<li><span>Long Term Disability fully paid for by organization</span></li>\n<li><span>Paid Time Off - 24 days in first year of employment</span></li>\n<li><span>Catastrophic Sick Time accrual</span></li>\n<li><span>7 Paid holidays</span></li>\n<li><span>Health Care and Dependent Care Flexible Spending Accounts</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif\">401k with employer match</span><br></li>\n<li><span>Student loan forgiveness eligible </span></li>\n</ul>\n<p><br><br></p>\n<p><span><em>We're an equal opportunity employer.
