Description

JOB DESCRIPTION - PNR SPEECH THERAPIST (SLP) REPORTS TO: Operations Manager JOB SUMMARY: Evaluates, plans, and administers comprehensive speech therapy modalities/regimens as prescribed by a physician to patients with speech, voice, and language, cognitive communicative and oral-pharyngeal swallowing disorders in their place of residence for the purpose of improving and/or restoring speech and/or swallowing functions. DUTIES AND RESPONSIBILITIES:  Performs accurate and comprehensive initial and ongoing speech, language and swallowing assessments in response to patients’ needs and in accordance with physician orders.  Initiates a written plan of care, re-evaluates and updates the plan as necessary.  Provides skilled physical therapy modalities within the therapy scope of practice.  Documents skilled care and/or services provided in compliance with federal and state laws and regulations, and Agency policies and procedures.  Participates in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, to assure patient care needs are met.  Addresses and supports patient/family cultural practices, as long as such practices do not harm others or interfere with the planned course medical therapy.  Promotes and provides patient/family/caregiver education using various verbal and written communication techniques that take into account the patient’s/family’s cultural, ethnic, and/or personal needs or preferences.  Supervises Speech Therapy Assistants in compliance with applicable laws, regulations and standards.  Plans patient discharge appropriately; provides information about community resources to address patient ongoing needs.  Knowledgeable of federal, state regulations and Agency’s policies and procedures regarding patient care. 215 Solutions for Care, Inc.(c) 2020  Communicates with all members of the health care team, including physicians appropriately and in a timely manner.  Notifies the physician, the Clinical Manager and other health care team members of any changes in the patient’s condition and the need to modify the plan of care.  Maintains currency of knowledge and skills.  Participates in the Agency’s Quality Assessment Performance Improvement program.  Attends Agency meetings and in-service programs as required.  Obtains appropriate number of continuing education credits to maintain re-licensure status.  Maintains confidentiality of patient and Agency information at all times. QUALIFICATIONS/PROFESSIONAL REQUIREMENTS:  Master’s Level Graduate in the area of Speech Therapy or Speech Language Pathology from an accredited university of college.  Certificate of Clinical Competence in Speech-Language Pathology awarded by the American speech Language and Hearing Association.  For Medicare Certified Agencies:  Meets the education and experience requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech-Language-Hearing Association; or  Meets the education requirements for certification and is in the process of accumulating the supervised experience required for certification.  Current valid state Speech Therapy license  Minimum of two (2) years’ experience as a Speech Therapist  Current CPR card  Current health certificate/physical examination and TB testing results  Valid state driver’s license and reliable automobile  Proof of current automobile insurance  Able to communicate effectively in English, both verbally and in writing. 216 Solutions for Care, Inc.(c) 2020  Additional languages an asset PHYSICAL DEMANDS:  While performing the functions of this job the employee is frequently required to stand, walk and sit, and use hands, use fingers, handle or feel.  The employee must be able to lift and/or move up to 50 pounds.
