This role involves collaborating closely with interdisciplinary healthcare teams, patients, and their families to support&nbsp;optimal&nbsp;physical function and independence within the comfort of the patient&#39;s home. The Physical Therapist also educates caregivers on effective techniques and safety measures, ensures compliance with all regulatory and organizational standards, and contributes to the holistic well-being and dignity of patients during all stages of care, including end-of-life support.&nbsp;</p><p>&nbsp;</p><p>Primary Duties and Responsibilities&nbsp;</p><ul><li><span data-contrast=\"none\"
