Working within a client- and family-centred model of care, the Household LPN assesses, plans, implements, and evaluates care while supporting residents' physical, social, spiritual, and psychological well-being.</p>\n<p> </p>\n<p>In this role, you will collaborate closely with residents, families, and healthcare team members to develop care plans, advocate for residents, and ensure individual preferences, abilities, and routines are reflected in daily care. We value the cultural richness that our diverse workforce brings to the care of our equally diverse population of patients and residents.</li>\n</ul>\n<p><strong>Your Day to Day</strong></p>\n<ul>\n <li>Provide safe and respectful nursing care, adhering to established policies, individualized care plans, and priorities</li>\n <li>Conduct thorough assessments, monitor signs/symptoms, and communicate with the RN/RPN as needed</li>\n <li>Prepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary team</li>\n <li>Administer medications per protocol, monitor effects, and maintain communication with the team</li>\n <li>Utilize evidence-based resources to stay updated on nursing practices</li>\n <li>Respond to incidents such as codes, injuries, spills, and alarms, following procedures</li>\n <li>Assist with staff orientation, demonstrating procedures and providing support</li>\n <li>Address patient/resident and family learning needs, provide educational materials, and make referrals</li>\n <li>Attend care planning meetings, provide input, and advocate for patients/residents</li>\n <li>Collaborate with the team to develop and evaluate individualized care plans</li>\n <li>Assist with discharge planning based on patient status and external supports</li>\n <li>Operate and clean equipment, report malfunctions, and handle hazardous materials.
