The Substance Abuse Counselor completes initial and annual substance abuse assessments for all clients, participates in the development of the initial service plan and its revisions, and provides individual, family and group substance abuse counseling and advocacy and referrals to community programs as needed. The Substance Abuse Counselor monitors clients&rsquo; attendance at programs and provides documentation of all contacts.&nbsp; <strong><em>This position is eligible for Comunilife&#39;s generous benefits and paid time off package including medical, dental, vision, life insurance, retirement employer contributions after 2 years, tuition reimbursement and professional development opportunities.</em></strong></p>\n\n<p>&nbsp;</p>\n\n<p><strong>Shift:&nbsp;</strong>Tues-Fri 11AM-7PM &amp; Sat 9AM-5PM</p>\n\n<p><strong><em>Recent grads are encouraged to apply</em></strong></p>\n\n<p><strong>Essential Duties:</strong></p>\n\n<ul>\n\t<li dir=\"ltr\">\n\t<p>Performs substance abuse assessments, collaborates with case managers to identify mutually agreed upon long and short-term substance abuse related goals and develop a comprehensive service plan.&nbsp;</p>\n\t</li>\n\t<li dir=\"ltr\">\n\t<p>Provides substance abuse counseling and support services to clients.</p>\n\t</li>\n\t<li dir=\"ltr\">\n\t<p>Facilitates weekly substance abuse groups including harm reduction, relapse prevention conflict resolution, etc.</p>\n\t</li>\n\t<li dir=\"ltr\">\n\t<p>Responsible for conducting initial intake interviews and assessments (as determined by site protocols) for all new clients and establishing case records.</p>\n\t</li>\n\t<li dir=\"ltr\">\n\t<p>Provides individuals and group clinical case management services for his/her caseload of Safe Haven clients.</p>\n\t</li>\n\t<li dir=\"ltr\">\n\t<p>Performs crisis intervention, assessment, advocacy, negotiation, referral, and case planning activities with Safe Haven clients as needed.</p>\n\t</li>\n\t<li dir=\"ltr\">\n\t<p>Train clients in activities of daily living skills.</p>\n\t</li>\n\t<li dir=\"ltr\">\n\t<p>Conducts unit/bed inspections, at least once a week and review the results of the inspections with the client.</p>\n\t</li>\n\t<li dir=\"ltr\">\n\t<p>Escorts clients to appointments (medical, entitlements, housing, etc.).</p>\n\t</li>\n\t<li dir=\"ltr\">\n\t<p>Serves as liaison with various agencies, schools, health care providers, mental health providers, entitlements, and employment and training providers in the interest of the clients.</p>\n\t</li>\n\t<li dir=\"ltr\">\n\t<p>Participates in weekly individual supervision with his/her supervisor, team meetings, housing/case reviews and case conferences.</p>\n\t</li>\n\t<li dir=\"ltr\">\n\t<p>Completes HRA 2010E applications.</p>\n\t</li>\n\t<li dir=\"ltr\">\n\t<p>Completes incident reports and submit to supervisor for review.</p>\n\t</li>\n</ul>\n",
