We proudly serve 30 cities across the Foothill, Pomona, and El Monte Health Districts within Los Angeles County. Our services span the full lifespan—from infants and toddlers, school-age children, and transition-age youth to adults and aging adults. At SG/PRC, our work is guided by compassion, collaboration, integrity, and a shared commitment to making a meaningful difference in the lives of the individuals and families we serve.<br>&nbsp;</p><p><strong>The Position</strong></p><p><br>This position reports to the Nurse Manager, and provides compassionate and dedicated nursing coordination, consultation, and support to Regional Center staff, service providers, clients, and families. Core responsibilities include assessments, evaluations, consultations, and delivery of high-quality care, ensuring individuals served receive appropriate medical care and that staff and families are well-informed and supported in managing health needs.<br>&nbsp;</p><p><strong>Essential</strong> <strong>Job Functions</strong></p><ul><li>Provide nursing coordination and consultation to Regional Center staff, service providers, clients, and families regarding health and medical needs within the nursing scope of practice.</li><li>Offer support and information related to medical terminologies, disease process, medical and surgical procedures, and other health-related topics to enhance understanding and care planning.</li><li>Conduct evaluation visits and develop comprehensive nursing assessments to determine levels of care, respite needs, significant changes in condition, and appropriate facility placement.&nbsp;</li><li>Support, monitor, and prepare monthly reports for the ARFPSHN homes to ensure compliance and quality of care.</li><li>Coordinate with SG/PRC Medical Consultant and Client Consultation Committee to facilitate effective communication and decision-making regarding client health and medical services. &nbsp;</li><li>Create educational presentations for staff, parents, providers, and community groups on health-related issues and disease prevention.</li><li>Coordinate with Quality Assurance Unit and Placement Committee to determine if appropriate medical care was provided by the facilities.</li><li>Conduct a nursing death review to support M&amp;M Committee in determining appropriate medical care provided by facilities.&nbsp;</li><li>Coordinate with local health care providers and hospitals regarding client health needs.</li><li>Act as an Executive Director Designee providing consents for medical and dental procedures for clients.</li><li>Performs other duties as assigned in support of departmental and organizational goals.</li></ul>",
