Legacy Link Traditional and Enhanced EDWP/ADRC Screening Specialist (RN) DEPARTMENT: ADRC Department ACCOUNTABILITY: Reports to ADRC Director STATUS: Exempt, Full-time- Hybrid schedule JOB SUMMARY The Traditional and Enhanced EDWP/ADRC Screening Specialist acts as the first point of contact for individuals requesting services though the Traditional and or Enhanced Elderly and Disable Waiver Program (EDWP), with for themselves or for someone else. The RN will be responsible for screening incoming potentially eligible Medicaid waiver clients. If the client does not meet the EDWP criteria, the RN will be responsible for referring the client to other programs or resources in their service area. The RN will also be responsible for the review of the Level of Care (LOC) pertaining to all clients assessed for EDWP as well as the issuance of any denials to the client where the client fails to meet the Medicaid criteria. DUTIES AND RESPONSIBILITIES Essential Functions Screens referrals within 5 business day of receipt. Accepts and screens all referrals from individuals wishing to participate in the Traditional/Enhanced EDWP. Conducts a telephone screening, completes the DON-R, and determines applicant priority for full assessment. Reviews potential Medicaid eligibility for Traditional/Enhanced EDWP. Identifies applicant needs and service requests as stated by the referral source. Notifies the potential applicant/referral source of referral status. Informs ineligible applicants the right to appeal. When appropriate, refers applicant for other available services. Determines if the applicant is eligible for Traditional/Enhanced EDWP and refers individuals on the waiting list to other appropriate resources. EDUCATION, EXPERIENCE AND SKILL REQUIREMENTS Skills: Ability to communicate with individuals, family members, caregivers, and social service agencies. Demonstrates knowledge and experience with health care and human services programs. Computer skills to a level that he/she can understand the state data base. Well organized and detail oriented Licensure and Certification Valid State Driver’s License Current GA Nurses License PHYSICAL AND MENTAL DEMANDS The physical and mental demands described are representative of those that must be met by an employee to successfully preform the essential functions of this job. Local Travel required Attendance at off-premises meetings, conferences or training’s. While performing the duties of this job, the employee is regularly required to use hands to finger, handle or feel and to walk, sit talk or hear. The employee is occasionally required to stand, climb or balance; stoop, kneel, crouch or crawl. The employee must occasionally lift and /or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. The noise level in the environment is moderate as phone conversations with the hard of hearing occur daily. WORK ENVIRONMENT The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of the job. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions of the job, on a case-by-case basis. This job operates in an office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Temperature ranges from normal indoor climate-controlled environment in buildings or vehicles, various outdoor conditions and temperature extremes encountered during off-site travel, and unpredictable indoor environmental conditions encountered during off-site travel, and unpredictable indoor environmental conditions encountered at off-site locations.
