Formulates and implements psychosocial treatment plans; identifies unmet psychosocial needs and develops programs to meet those needs. Participates in educational activities, professional organizations and hospital committees.</p>\n<p>Job Responsibility</p>\n<p></p>\n<ul>\n <li>Promotes Social Work Services department goals by selecting, motivating, and training capable team members.</li>\n <li>Leads the activities of assigned Social Work Services team members by communicating and providing guidance toward achieving department objectives. Provides management and leadership to team members of Social Work Services by communicating and guiding toward achieving department objectives. Develops, communicates, and builds consensus for goals in alignment with system.&nbsp;</li>\n <li>Promotes department goals by selecting, motivating, and training capable team members.&nbsp;</li>\n <li>Leads the activities of assigned team members by communicating and providing guidance toward achieving department objectives.&nbsp;</li>\n <li>Assists in screening applicants for social work positions.&nbsp;</li>\n <li>Assists staff in formulating and implementing psychosocial treatment plans based on patient and family needs.&nbsp;</li>\n <li>Meets with staff social workers to review caseloads; assists staff in examining clinical, administrative and ethical issues.&nbsp;</li>\n <li>Provides clinical supervision as applicable.&nbsp;</li>\n <li>Monitors time, charting and statistics of social work staff in accordance with departmental criteria; participates in interdisciplinary treatment planning conferences.&nbsp;</li>\n <li>Performs psychosocial assessments of patients and families; identifies patients needing psychosocial services.&nbsp;</li>\n <li>Performs competency assessment/performance appraisals according to institutional schedules and policies.&nbsp;</li>\n <li>Interviews patients and families, when necessary, to assess family structure, dynamics, adjustments to illness and access to necessary resources; identifies medical problems and predicts course; educates patients and families about community resources and discharge and planning procedures.&nbsp;</li>\n <li>Participates in internal and external committees and workgroups that require social work expertise and best practice input.&nbsp;</li>\n <li>Performs related duties as required. All responsibilities noted here are considered essential functions of the job under the Americans with Disabilities Act.
