    "requisitionDescription": "<div><div><div><p><br></p><table border=\"1\" cellspacing=\"0\" cellpadding=\"0\" width=\"638\" id=\"isPasted\"><tbody><tr><td valign=\"top\" style=\"width: 100%;\"><p><strong>Role and Responsibilities</strong></p><p>A qualified clinical professional who directs, supervises, evaluates and provides nursing services to patients in the home, as prescribed by the attending physician. The Lead RN Nurse Preceptor is recognized as an experienced nurse whose high level of home health proficiency is clearly evident to peers, patients, and referral sources.</p><p>&nbsp;<strong>Essential Functions</strong></p><p>In addition to the essential functions of an RN Case Manager, the Lead RN Nurse Preceptor may be required to:</p><ul><li>Provide in-home clinical orientation for new employees or current employees needing assistance by providing 1:1 teaching and modeling of job duties through co-visits.</li><li>Deliver didactic classroom training, as needed.</li><li>Evaluate and document skill performance of preceptee to determine skill competency.</li><li>Coordinate with new clinical employees through orientation period to provide mentoring, training and advocacy.</li><li>Participate in case conference/staff meetings with preceptee and manager weekly during orientation period.</li><li>Become an instructor through the DOH for the HHA training program.</li><li>Participates in pilot programs as needed to assist in the quality and growth within the CHHA or other areas as they arise.</li><li>Preform Supervisory co-visits as needed for the field staff.</li><li>Participate in Competency days throughout the footprint.</li><li>Other duties as assigned.</li></ul><p>&nbsp;This job description reflects management&rsquo;s assignment of essential functions; and nothing in this herein restricts management&rsquo;s right to assign or reassign duties and responsibilities to this job at any time.</p><p>&nbsp;<strong>Education Requirements</strong></p><ul><li>Associate&rsquo;s degree in Nursing required.</li><li>BSN degree preferred.</li></ul><p><strong>&nbsp;</strong><strong>Qualifications and Requirements</strong></p><ul><li>Current, valid RN license issued by NYS Department of Education and minimum one year home care experience.</li><li>Knowledge of State and Federal regulations, as well as payer guidelines.</li><li>Current, unexpired NYS Driver&rsquo;s License.</li><li>Current CPR Certification.</li><li>Minimum 2 years of home care experience</li></ul><p>&nbsp; &nbsp; Must possess the following skills:</p><ul><li>Demonstrate critical thinking skills.</li><li>Strong organizational and time management skills, possesses ability to multitask.</li><li>Strong documentation skills.</li><li>Clinical competency of nursing skills.&nbsp;</li><li>Demonstrated effective communication and collaboration with multidisciplinary care team.</li></ul><p>&nbsp;<strong>Work Environment</strong></p><p>The RN Nurse Preceptor is primarily a non-office setting and may be exposed to outdoor conditions.</p><p>The working conditions are classified as medium work:</p><ul><li>Medium work &ndash; Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.
