Together, we are making a difference, one life at a time. </p>\n<p> </p>\n<p><strong>Your Mission in Action</strong></p>\n<p>The Medical Coordinator arranges and coordinates appropriate health services, including urgent, emergent, and preventative health services, for children in care. This includes maintaining complete, confidential medical records for children, ensuring appropriate communication of health needs to case managers and clinicians, scheduling health care appointments, coordinating transportation to health care appointments, ensuring qualified interpretation and translation services are in place, and serving as a liaison for community health care providers. This role adheres to agency policies, licensing, and regulatory standards.  </p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Provide UC with routine health screenings and perform basic health assessment; assess each UC’s unique medical needs and ensure they are properly met in a timely manner; report routine medical needs to the medical, clinical, case management and direct care staff per Agency policy and licensing standards </li>\n <li>Administer medication and treatment to UC as prescribed or as needed in accordance with licensing standards and nursing practices; coordinate medical appointments and follow through on medications and treatment as prescribed; coordinate and oversee the prescribed nutritional needs and diet and fluid intake for UC as prescribed </li>\n <li>Facilitate trainings for staff, such as dispensing prescribed, over the counter and psychotropic medication, blood borne pathogens, and CPR/First Aid </li>\n <li>Create, update, and maintain accurate and up-to-date UC case files and records with information pertaining to nursing care, child’s medical history, treatment plans, and observations; chart all pertinent information in the UC case files/records in accordance with Agency policy and licensing standards </li>\n <li>Complete monthly, quarterly, and annual reports as requested and to remain in compliance with licensing and funding standards </li>\n <li>Assist with the assessment, development, and evaluation of a UC individualized service plan based on their needs and medical condition; use care plans as a basis for providing therapeutic care to UC </li>\n <li>Respond to all crisis situations in a timely manner; ensure all Program departments are properly notified in a timely manner; report incidents according to licensing standards </li>\n <li>Assist with the safe and physical evacuation of UC during an emergency or natural disaster  </li>\n <li>Role includes on-call responsibilities as schedule and as medical emergencies/needs may arise; consultation can be provided remotely or on-site based on need </li>\n <li>Work as a team player and exhibit professional behavior and a positive attitude with both internal and external clients that reflects positively on the Agency and is consistent with Agency policies and practices </li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Minimum Qualifications</strong></p>\n<ul>\n <li>Current LPN/LVN license in Texas or higher health–related qualification (e.g. licensed resisted nurse (RN) with bachelor’s degree) OR Bachelor’s degree in relevant field</li>\n <li>One (1) year of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care </li>\n <li>High School diploma or equivalent  </li>\n <li>Bilingual English and Spanish, (speak, read and write) </li>\n <li>Must be 21 years of age or older per licensing requirements</li>\n <li>Ability to maintain a professional approach and build a rapport and trust with residents and their families </li>\n <li>Consistently follows established standards, policies, and procedures in providing nursing care </li>\n <li>Strong writing skills with the ability to document and maintain accurate UC files/records </li>\n <li>Strong customer service, interpersonal, and people skills;
