Description We’re looking for a motivated and compassionate Mental Health Liaison to support individuals transitioning from Nursing Facilities for Mental Health (NFMH) back into the community. In this role, you’ll work closely with facility staff, community providers, and patients to ensure smooth, well-supported discharges and reduce repeat hospitalizations or involvement with the justice system. If you’re someone who can build relationships, navigate systems, and follow through—this is work that truly matters. What You’ll Do Case Management & Patient Support Provide strengths-based case management services in the community Build rapport with new patients through outreach and consistent engagement Assess needs, identify strengths, and set goals with patients (and supports when appropriate) Help patients problem-solve barriers related to housing, services, and daily living Connect individuals to appropriate mental health, medical, and community resources Encourage community involvement and increased independence Provide advocacy to help patients access services and protect their rights Assist with crisis intervention when needed Discharge Planning & Coordination Collaborate with NFMH staff, families, and community partners to support discharge planning Maintain regular communication with all parties involved in patient care Coordinate housing and outpatient services prior to discharge Notify providers of pending discharges and ensure continuity of care Communicate progress and barriers to leadership Provide required updates to state partners (KDADS) Documentation & Administrative Responsibilities Complete timely and accurate documentation (within 72 hours) Participate in treatment team meetings and case consultations Manage schedule and priorities based on patient and program needs Complete required state reporting (e.g., AIMS) Assist with special projects and community education as needed What You Bring Experience in case management, behavioral health, or social services Strong understanding of discharge planning and community-based services Ability to coordinate across multiple systems (healthcare, housing, social services) Strong communication and relationship-building skills Ability to stay organized and manage a flexible, community-based schedule Sound judgment in high-stress or complex situations What Success Looks Like Patients transition safely into the community with services in place Reduced readmissions to hospitals or involvement with the justice system Strong collaboration with internal teams and external partners Consistent follow-through and timely documentation Professional Expectations Participate in supervision, team meetings, and quality improvement initiatives Maintain confidentiality and uphold patient rights at all times Demonstrate professionalism, accountability, and a solutions-focused mindset Contribute to a positive, team-oriented work environment Why This Role Matters This work directly impacts whether someone succeeds or struggles after leaving a facility. Qualifications Minimum Education :  Bachelor’s degree in related field of behavioral sciences or equivalently qualified by work experience or a combination of work experience in the human services field and education Minimum Experience :  Experience working with individuals experiencing severe and persistent mental illness, or experience working with individuals who are discharging from a hospital, nursing home, etc. preferred. Must be at least 18 years of age Must possess demonstrated interpersonal skills and be able to relate to patients empathetically and non-judgmentally Must have good listening skills, verbal skills, and written communication skills Must be able and willing to be flexible, a self-starter, punctual, and have the ability to maintain a schedule Must be willing to work independently Must have good problem solving skills Must have or be willing to acquire factual knowledge about mental illness, its symptoms, treatment, and rehabilitation Must possess leadership skills and the ability to coordinate services Must be able to carry out and demonstrate living skills, such as food preparation, laundry, housekeeping, personal hygiene, etc. Must be able to complete required training within appropriate time frames Must be proficient at basic keyboarding/word-processing skills Must be able to pass Kansas Bureau of Investigation Screen and the Central Registry Screen for both adults and children and other company required background checks.
