Ensure Plan Implementation – Monitor and ensure that ISPs are being effectively carried out. Case Management – Provide linkage to resources and case management support for individuals. Timely Documentation – Submit necessary documentation according to established timeframes. Collaborative Approach – Maintain continuous communication with individuals and key stakeholders (with signed authorization) to ensure coordinated planning and service delivery. Additional Duties – Complete other responsibilities as assigned to support the overall success of the program. Qualifications: Educational Requirement: Bachelor’s degree or higher in a Human Service field Licensing: Valid registration with the Virginia Board of Counseling as a Qualified Mental Health Professional (QMHP), Qualified Mental Health Professional-Trainee (QMH P-T), or QPPMH (with certificate). Background Check: Successful completion of criminal background and central registry checks. Driving Requirements: Valid Driver’s License, current auto insurance, and reliable  transportation. Health and Safety Certification Requirements: Current TB screening and CPR/First Aid certification  (BLS not accepted)  or willing to obtain prior to start date. Skill Building & Caregiver Support: Ability to provide various skill-building activities, including community and daily living skills, while enhancing caregiver roles and assisting individuals in developing positive relationships and coping strategies. Behavior, Symptom & Crisis Management: Ability to assist individuals in developing behavior and anger management techniques, monitor and manage mental health symptoms, and effectively handle crisis situations, maintaining composure and professionalism even with hostile or volatile individuals. Confidentiality:  Ability to maintain confidentiality and comply with Human Rights and HIPAA regulations. Documentation:  Ability to develop and write detailed progress notes and reports, documenting direct or collateral contacts. Aftercare Planning: Ability to develop and implement closure and aftercare plans for individuals and families. Work Location:   Mobile
