Responsible for informing the community and creating awareness about Borinquen Medical Centers (BMC) services, including: substance use, mental health, HIV prevention, and prevention education on the risks of substance misuse, HIV/AIDS, and STDs. &nbsp;The Behavioral Health Outreach Specialist also conducts outreach in high-risk areas to recruit participants for testing and BMC services, provides needed linkages to service provision for individuals with mental health, substance use and/or co-occurring disorders, and participates in the coordination and facilitation of presentations to educate the community about our services.&nbsp;</p><p><strong>TASKS AND RESPONSIBILITIES</strong></p><ol><li id=\"isPasted\">Assist Project Director and Behavioral Health Lead Navigator in SAMHSA annual reports, continuation applications, etc. for Behavioral Health Department. &nbsp;</li><li>Assist Project Director and Behavioral Health Lead Navigator with integration of behavioral health services and communication between disciplines (psychiatrists, physicians, APRNs, LCSWs, LMHCs, etc.).&nbsp;</li><li>Attend weekly staff meetings, Behavioral Health Department meetings, and BMC general staff meetings.</li><li>Communicate effectively with supervisor regarding caseloads, issues regarding supervision, and leave requests.&nbsp;</li><li>Conduct street and community outreach during working hours, including some nights and weekends as needed. &nbsp;</li><li>Assist Project Director and Behavioral Health Lead Navigator in the creation, scheduling, and presentation of education sessions/groups for HIV, STD awareness, mental health, Life Skills, substance use, and other BMC services. &nbsp;&nbsp;</li><li>Distribute preventive materials including safe sex kits, condoms, and flyers.</li><li>Educate target population, newly diagnosed HIV persons, and others about BMC&rsquo;s available services, treatment options, and assist with linkages to services.</li><li>Engage and educate individuals aged 13 and up in risk-reduction discussions.</li><li>Ensure all certifications required for your position remain active and that updated copies are submitted for your employee file. &nbsp; &nbsp;</li><li>Ensure data collection is completed in a timely, accurate and consistent manner, including HIV and STD testing, follow-up questionnaires, and others.</li><li>Make home visits and reach out to individuals that are high risk, hard to reach, and/or present an adherence problem to treatment.</li><li>Navigate coordination and expedite services and linkages to additional wrap around care.&nbsp;</li><li>Perform behavioral screenings and assessments including GPRA, questionnaire data, demographic data collection instruments, pre-post training assessments, and surveys of partners and staff. &nbsp;</li><li>Assist Project Director and Behavioral Health Lead Navigator in performing periodic review of performance measures to assess progress toward goals, inform program development, and enable fine-tuning of implementation strategies.&nbsp;</li><li>Present at internal departmental meetings and educate BMC staff about programs and services</li><li>Provide HIV, hepatitis, and STD testing and counseling.</li><li>Provide program with community referrals, providing name and location of available van and outreach sites.&nbsp;</li><li>Recruit participants for all Behavioral Health programs, HIV/hepatitis/STD testing, and prevention services.</li><li>Work with the Marketing Department to create campaigns and projects for social media.</li><li>Perform all other duties as assigned by Director of Behavioral Health and/or Behavioral Health Program Managers.&nbsp;</li></ol><p><br></p><p><strong>REQUIREMENTS</strong></p><ol><li id=\"isPasted\">Minimum High School Diploma, or equivalent.</li><li>Outreach experience.</li><li>Bilingual preferred (Spanish/English or Haitian-Creole/English).&nbsp;</li><li>Strong interpersonal skills.&nbsp;</li><li>Valid FL driver&rsquo;s license in good standing.</li><li>HIV/AIDS 500-501 from DOH.</li></ol><p><br></p></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div></div>\n",
