Global Dynamics, LLC is a leader in the personnel support services industry setting a new standard in career placement and flexible staffing.</p>\n\n<p>Position Purpose: The Pharmacist is responsible for educating, assisting, and interacting with patients in an outpatient clinic setting with regards to health promotion and wellness.</p>\n\n<p>Duties and Responsibilities:</p>\n\n<ul>\n\t<li>&nbsp;Works collaboratively with multi-disciplinary teams to optimize drug therapy.</li>\n\t<li>&nbsp;Provide direct patient care, including treatments assessment and medication management activities.</li>\n\t<li>&nbsp;Clinically evaluate all patients in a specified area daily.</li>\n\t<li>&nbsp;Ensure documentation is placed in the medical record regarding pharmacy recommendations, plans, intervention and medication reconciliation in medical records.</li>\n\t<li>&nbsp;Review patient&rsquo;s drug regimen for an appropriate indication, dosing, allergy verification, potential drug interactions, lab monitoring, duplication of therapy, and medical documentation.</li>\n\t<li>&nbsp;Provide accurate and timely drug information to physicians, nurses and other health professionals.</li>\n\t<li>&nbsp;Respond to level 1 trauma, code blue massive blood transfusion emergencies.</li>\n\t<li>&nbsp;Provide direct patient care and medication therapy monitoring, including antibiotic stewardship, pharmacokinetic drug dosing, renal and hepatic dosing and patient education.</li>\n\t<li>&nbsp;Develop, implement, and monitor clinical protocols.</li>\n\t<li>&nbsp;Provide consultative services and clinical expertise to evaluate and improve medication usage and therapeutic outcomes.</li>\n\t<li>&nbsp;Process orders and prescriptions to provide medication and pharmaceutical supplies ordered by physicians and authorized medical practitioners.</li>\n\t<li>&nbsp;Facilitate discharge planning and smooth transition to alternate sites for care from a pharmacy perspective.</li>\n\t<li>&nbsp;Provide cost-effective management of drug utilization within the clinic.</li>\n\t<li>&nbsp;Monitor polypharmacy and high dose pharmaceutical dosing to prevent adverse effects.</li>\n\t<li>Provide drug information to patients and their families.</li>\n\t<li>&nbsp;Demonstrate professional communication with patients, family members, professional and support personnel involved in the delivery of patient care.</li>\n</ul>\n",
