In this role, you’ll support patients - by assessing mental health needs, developing care plans, and collaborating with a multidisciplinary team to ensure well-rounded support for conditions such as addiction, depression, and anxiety.<br><br></p>\n<p><span style=\"font-weight: bold\">Key Responsibilities: </span></p>\n<ul>\n<li>Conduct thorough biopsychosocial assessments and develop personalized care plans.</li>\n<li>Focus on treating addiction, depression, and anxiety, primarily in senior populations.</li>\n<li>Liaise closely with the medical team to define care goals and manage patient outcomes.</li>\n<li>Educate patients about available services and promote active participation in their care.</li>\n<li>Maintain timely and accurate documentation in the Electronic Health Record (EHR).</li>\n<li>Participate in weekly clinical supervision and interdisciplinary team meetings.</li>\n<li>Uphold NASW ethical and professional standards in all areas of practice.</li>\n<li>Build therapeutic rapport with patients from diverse backgrounds while maintaining clinical boundaries.</li>\n<li>Responsible for outreach to Pod Health clients for enrollment in the Behavioral Health Integration (BHI) program</li>\n<li>Conduct outreach and follow-up calls with clients experiencing crisis situation</li>\n<li>Follow up on referrals received from the medical team to coordinate behavioral health support and services</li>\n<li>Schedule and conduct therapy sessions for clients through Zocdoc.</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Required Qualifications:<br></span></p>\n<ul>\n<li>Master’s degree in Social Work from an accredited institution.</li>\n<li>Current LCSW license in New York State required.</li>\n<li>Bilingual Spanish required.</li>\n<li>Experience supporting patients with addiction, depression, and anxiety.</li>\n<li>Strong interpersonal, communication, and documentation skills.</li>\n<li>Ability to work independently and collaboratively within a team-based model.</li>\n<li>Commitment to HIPAA compliance and maintaining confidentiality.</li>\n<li>Passion for serving aging and diverse communities.<br><br></li>\n</ul>\n<p><span><span style=\"font-weight: bold\">Preferred Qualifications:</span></span></p>\n<ul>\n<li>Familiarity with telehealth platforms or virtual care settings.<br><br></li>\n</ul>\n<p><span style=\"font-weight: bold\">Benefits:</span></p>\n<ul>\n<li>Health, dental, and vision insurance</li>\n<li>401(k) with company match</li>\n<li>Paid Vacation, Company Holidays, Sick Leave, and Floating Holidays<br></li>\n<li>Professional development opportunities</li>\n<li>Mission-driven work in an innovative healthcare space</li>\n</ul>",
