Salary will be $44,000 per year, includes insurance and time off benefits Full time position working 40 hours per week, 9:00 am to 5:00 pm Easterseals Florida is seeking a compassionate and dedicated Family Service Coordinator (FSC) to support families of young children eligible for IDEA, Part C services through our North Beaches Early Steps program. The FSC plays a vital role in empowering families, helping them understand their child’s strengths, needs, and available supports, and serving as the central point of contact from the start to the end of their Early Steps journey. In this role, you will work with a comprehensive team of professionals to ensure families receive timely evaluations, services, procedural safeguards, and coordinated care. You will serve as a supportive, communicative, and responsive resource to families, helping them navigate resources and advocating for services that promote optimal child development. What You’ll Do Provide ongoing service coordination for children eligible for Early Steps as the assigned service coordinator Participate in multidisciplinary evaluations, IFSP meetings, and transition planning activities Support eligibility determinations, monitor interventions, and assist families through transitions from IDEA Part C Promote and strengthen the IFSP team’s ability to identify, access, coordinate, and evaluate services and resources Interview families to gather information on routines, strengths, needs, support systems, and developmental concerns Develop timely and comprehensive Individualized Family Support Plans (IFSPs) in collaboration with families and team members Communicate IFSP recommendations with providers, coordinate updates every 6 months, and ensure follow-up for developmental evaluations Coordinate transitions to Part B for families choosing school-based services, including required notifications and planning Assist families not pursuing Part B with developing an appropriate post-Part C plan Maintain accurate case notes, file documentation, and billing practices for CMS Targeted Case Management Travel to family homes or preferred locations to conduct interviews, gather information, and complete coordination tasks What we offer: Live and on demand Professional Development opportunities Medical, Dental, and Vision Plans Paid Holidays, Vacation, Sick, and Personal Time Employee Assistance Program Several Supplemental Insurance Policies 403B Savings Plan Easterseals Cares Wellness Program Work Life Balance The Opportunity to Make a Difference in the Community and the Organization Qualifications Bachelor’s degree from an accredited university in psychology, social work, early childhood, child development, special education, health education, sociology, or a related field. Ability to secure and maintain a Medicaid Provider Number. Knowledge of community resources, child development, disabilities, and confidentiality/privacy laws. Strong documentation, time-management, and organizational skills. Ability to travel to family homes and adjust schedule to meet family needs. Ability to provide crisis management support when necessary. Demonstrates integrity, tact, strong judgment, flexibility, and a commitment to self-development and quality practices. Commitment to teamwork and to supporting families with empathy and professionalism.
