This position does initial assessments, and regular re-assessments: provides ongoing nursing services, works as part of an interdisciplinary team, focuses on prevention, early detection, acute and chronic illness. In the event an RN from a physician practice works in an inpatient area, s/he will have the capability to write verbal orders, enter orders, and pend access notes, and will be required to attend physician THC training. Job Responsibilities: Plans care for participants and family based on assessment, standards of care and optimal outcomes. Initiates and individualizes appropriate patient care guidelines, POC or clinical pathways. Maintains current plan of care by updating as needed. Develops both short and long term goals. Evaluates the plan of care based on optimal patient outcomes. Documents the participant / family response to care including teaching. Collaborates with other members of the health care team to revise plan of care as needed. Supervises the care that was delegated to other health care team members. Demonstrates organizations responsibilities: Identifies areas for self improvement, functions in relief charge / resource role as requested, completes assignments within scheduled timeframes, cooperates with instructor to facilitate effective learning experiences for students, maintains current knowledge in area of practice, demonstrates knowledge of organizational and unit changes.
