In British Columbia, mNPRs are developed in partnership with the Ministry of Health, the BC Nurses’ Union (BCNU) and health organizations. Job Summary Come work as a Home Health Triage Nurse with Vancouver Coastal Health (VCH) at Groundspring Home Health in Vancouver, BC! Apply now to join the team! Within the context of a client and family-centred framework and continuum-based care model and in accordance with established BCCNM professional and practice standards, and the vision, values and policies of the organization, the Home Health Triage Nurse provides initial client screening related to referral priority and appropriateness of referrals, urgency of needs, and confirmation of the appropriate required service. Acts as a liaison with other internal and external teams to assess Home Health nursing care needs for clients. Coordinates the site-specific nursing assignments based on priority, ensure workload is distributed evenly, and client care needs are met. Follows established standardized client allocation/assignment processes and utilizes established escalation procedures to ensure timely and appropriate client allocation/assignment so as to not delay care provision. Prioritizes daily workload to accommodate urgent client needs and referrals. As required provides direct client care by assessing, planning, coordinating, organizing, implementing, evaluating, and transitioning nursing and other services for the client. Assesses service eligibility, facilitates the development, implementation, and adaptation of a care plan, and provides care management to assist clients to manage their own care and navigate through the various services available within the community settings. Works in consultation with other system partners and incorporates current evidence into practice.This work is framed within VCH’s commitments to our pillars of Indigenous Cultural Safety, Anti-Racism, Equity, Diversity and Inclusion, and Planetary Health. About Home Health Programs: Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative or rehabilitative health care needs. Qualifications Education & Experience Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). One-year recent related clinical experience in Home Health working with complex clients/patients with chronic health conditions and/or functional mobility issues including recent experience in care and transition planning, or an equivalent combination of education, training and experience. Valid BC Driver’s License required as local area travel may require the use of a vehicle or other accepted mode of transportation. Knowledge, Skills & Abilities Demonstrated knowledge and skills in community health nursing theory and practice within a client and family centred model of care. Demonstrated ability to triage multiple clients’ needs and prioritize urgent situations and escalate level of care as appropriate. General knowledge of Trauma Informed Practice and Harm Reduction approaches and Strengths-based care. Demonstrated ability to promote Indigenous Cultural Safety. Demonstrated ability to provide competent and culturally safe care in a variety of settings and with diverse populations. Demonstrated ability to assess safety and risk prior to entering familiar and unfamiliar home environments and to integrate and evaluate pertinent data (from multiple sources) to problem-solve effectively. Demonstrated ability to manage psychosocial and behavioral issues, and employ effective conflict resolution and reconciliation approaches, techniques, and strategies related to goals of care, transitions and end of life care. Demonstrated ability to manage and prioritize clients and promote cooperation, assertiveness, creative planning for change and innovations, implementation of policies or other protocols and ongoing professional development of self. Demonstrated ability to adjust to new or unexpected events and promote client-focused care with clients and significant others, sensitive to diverse cultures and preference, client advocacy and social justice concerns. Demonstrated ability to work collaboratively as a member of an interdisciplinary team with clients/caregivers to holistically assess/plan/monitor a wide variety of health challenges in the home/community setting and transitions through care continuums Demonstrated ability to document timely and appropriate information in an electronic medical record (EMR), reporting assessments, decisions about client status, plans, interventions and client outcomes. Ability to effectively use related computer software, applications, and devices. Demonstrated ability to maintain client/caregivers’ privacy and confidentiality with respect to communication, documentation, and data, including when travelling between client homes and office/care settings Demonstrated ability to develop rapport, trust and ethical relationships with clients/families, family care providers and other health care professionals; maintains professional boundaries in relationships in home and community settings. Ability to communicate effectively through verbal, nonverbal, written, electronic and social media. Demonstrated ability to teach, facilitate and coach employing teaching/learning strategies, adult education principles, methods and tools to transfer knowledge and engage clients/families in planning and self-management of care Knowledge of provincial acts, regulations, and program policies and guidelines related to home and community care. Knowledge of acute, chronic disease, palliative, mental health and substance use and health management and self-management support. Knowledge of community resources available for client/family/caregiver support and related health services. Knowledge of research, quality improvement, evaluation process and methodologies. Demonstrated ability to plan, organize, schedule, multitask and prioritize work. Demonstrated skill in CPR techniques. Demonstrated skill in the use of equipment and in the techniques appropriate to nursing treatment. Broad knowledge of the BCCNM standards of practice and guidelines for clinical practitioners. Ability to operate related equipment including relevant computer applications. Demonstrated physical ability to perform the duties of the position. For many, it has the potential to be a doorway to belonging, stability, and healing, and for our teams, a place where meaningful, relationship-based care can continue to grow.</p>\n<p> </p>\n<p><strong>About Home Health Programs:</strong></p>\n<p> </p>\n<p>Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative or rehabilitative health care needs.</p>\n<p> </p>\n<p></p>\n<p> </p>\n<p></p>\n<h2>Qualifications</h2>\n<p><strong>Education & Experience</strong></p>\n<ul>\n <li>Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).</li>\n <li>One-year recent related clinical experience in Home Health working with complex clients/patients with chronic health conditions and/or functional mobility issues including recent experience in care and transition planning, or an equivalent combination of education, training and experience.</li>\n <li>Valid BC Driver’s License required as local area travel may require the use of a vehicle or other accepted mode of transportation.</li>\n</ul>\n<p><strong>Knowledge, Skills & Abilities</strong></p>\n<ul>\n <li>Demonstrated knowledge and skills in community health nursing theory and practice within a client and family centred model of care.</li>\n <li>Demonstrated ability to triage multiple clients’ needs and prioritize urgent situations and escalate level of care as appropriate.</li>\n <li>General knowledge of Trauma Informed Practice and Harm Reduction approaches and Strengths-based care.</li>\n <li>Demonstrated ability to promote Indigenous Cultural Safety.</li>\n <li>Demonstrated ability to provide competent and culturally safe care in a variety of settings and with diverse populations.</li>\n <li>Demonstrated ability to assess safety and risk prior to entering familiar and unfamiliar home environments and to integrate and evaluate pertinent data (from multiple sources) to problem-solve effectively.</li>\n <li>Demonstrated ability to manage psychosocial and behavioral issues, and employ effective conflict resolution and reconciliation approaches, techniques, and strategies related to goals of care, transitions and end of life care.</li>\n <li>Demonstrated ability to manage and prioritize clients and promote cooperation, assertiveness, creative planning for change and innovations, implementation of policies or other protocols and ongoing professional development of self.</li>\n <li>Demonstrated ability to adjust to new or unexpected events and promote client-focused care with clients and significant others, sensitive to diverse cultures and preference, client advocacy and social justice concerns.</li>\n <li>Demonstrated ability to work collaboratively as a member of an interdisciplinary team with clients/caregivers to holistically assess/plan/monitor a wide variety of health challenges in the home/community setting and transitions through care continuums</li>\n <li>Demonstrated ability to document timely and appropriate information in an electronic medical record (EMR), reporting assessments, decisions about client status, plans, interventions and client outcomes. Ability to effectively use related computer software, applications, and devices.</li>\n <li>Demonstrated ability to maintain client/caregivers’ privacy and confidentiality with respect to communication, documentation, and data, including when travelling between client homes and office/care settings</li>\n <li>Demonstrated ability to develop rapport, trust and ethical relationships with clients/families, family care providers and other health care professionals; maintains professional boundaries in relationships in home and community settings.</li>\n <li>Ability to communicate effectively through verbal, nonverbal, written, electronic and social media.</li>\n <li>Demonstrated ability to teach, facilitate and coach employing teaching/learning strategies, adult education principles, methods and tools to transfer knowledge and engage clients/families in planning and self-management of care</li>\n <li>Knowledge of provincial acts, regulations, and program policies and guidelines related to home and community care.</li>\n <li>Knowledge of acute, chronic disease, palliative, mental health and substance use and health management and self-management support.</li>\n <li>Knowledge of community resources available for client/family/caregiver support and related health services.</li>\n <li>Knowledge of research, quality improvement, evaluation process and methodologies.</li>\n <li>Demonstrated ability to plan, organize, schedule, multitask and prioritize work.</li>\n <li>Demonstrated skill in CPR techniques.</li>\n <li>Demonstrated skill in the use of equipment and in the techniques appropriate to nursing treatment.</li>\n <li>Broad knowledge of the BCCNM standards of practice and guidelines for clinical practitioners.</li>\n <li>Ability to operate related equipment including relevant computer applications.</li>\n <li>Demonstrated physical ability to perform the duties of the position.</li>\n</ul>\n<p> </p>\n<h2>Closing Statement</h2>\n<p>The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks.
