For over 80 years, TCC Family Health has been dedicated to eliminating barriers to care, to improve health and lower health systems cost by expanding cost-effective, meaningful and innovative health care programs that work and providing comprehensive healthcare in a culturally sensitive and linguistically appropriate manner to medically underserved, low-income and high-risk populations in Long Beach and surrounding communities.</p><p style=\"margin-left:0in;\"><strong>Job Summary:&nbsp;</strong></p><p style=\"margin-left:0in;\">The Outreach Specialist role focuses on building trust and is responsible for performing a variety of tasks including outreach, engagement, screening, and enrollment of expectant and new parents into the Healthy Families America (HFA) program, while supporting broader Health Education &amp; Outreach Department initiatives. The Outreach specialist is an important participant in the continuum of care and will contribute to health promotion and disease prevention by providing support to programs in the Health Education &amp; Outreach Department, connecting individuals and families to appropriate classes, health access, preventive care, oral health services, 0-5 programming, behavioral health support and other allied health services to improve health outcomes.</p><p style=\"margin-left:0in;\">The Outreach Specialist is an integral member of the health education and outreach team and a leader in the community. The Outreach Specialist works closely with medical providers, TCC staff from various clinics and programs, patients, community members, volunteers and other organizations in the community. This person must be able to communicate effectively and professionally within the health care environment as well as with the residents in the community. The Outreach Specialist must be willing to perform community-based activities in order to achieve the larger goals and objectives of the Health Education and Outreach Department, as it serves the mission of The Children&rsquo;s Clinic, &ldquo;Serving Children and Their Families&rdquo;.</p><p style=\"margin-left:0in;\"><strong>Qualifications:</strong></p><div style=\"margin-left:0in;\"><ul style=\"list-style-type: disc;\"><li style=\"margin-left:0in;\">Bachelor&rsquo;s degree required</li><li style=\"margin-left:0in;\">Experience in and understanding of effective primary prevention</li><li style=\"margin-left:0in;\">Experience working with diverse populations and communities with limited access to care</li><li style=\"margin-left:0in;\">Bilingual Spanish/English or Khmer/English required</li><li style=\"margin-left:0in;\">Experience in coalition work at the local or state level desired</li><li style=\"margin-left:0in;\">Excellent speaking, writing and analytical skills&nbsp;</li><li style=\"margin-left:0in;\">Ability to provide respectful, empathetic and excellent customer service</li><li style=\"margin-left:0in;\">Ability to coordinate multiple projects and deadlines in a high-quality manner with attention to detail&nbsp;</li><li style=\"margin-left:0in;\">Ability to work and thrive in a fast-paced, dynamic environment;
