Description

Description Job Description: The Kentucky Moms MAT-R Case Manager holds the primary responsibility for evaluating individuals’ needs and coordinating care and services as needed/requested. They also provide consultation and training to other partner agencies while marketing the program to potential referral sources. Participation in workshops, facilitating training, attending resource fairs and healthy baby workshops, etc. is expected. This position works collaboratively with the ROSES for Moms program and other Comprehend programs. Role and Responsibilities 1. Completes a written, comprehensive needs assessment for identified clients upon completion of intake and determining level of care utilizing standardized assessments. 2. Maintains face-to-face in-home/community contacts. 3. Provides assistance in the development of the client’s treatment plan. 4. Assists the client in accessing services from other agencies/organizations. 5. Monitors client’s progress towards treatment plan/goals. 6. Provided case consultation and crisis assistance as needed/required. 7. Participate in process improvement activities as directed by program needs/deliverables. 8. Complete service documentation and billing entries within 48 hours of service delivery. 9. Attend statewide Kentucky Moms meetings, trainings, symposiums, etc. as scheduled. 10. Enter all service data in the CDAR Data System including intake/baseline surveys and case management activities. 11. Fosters collaborative working relationships among members of the leadership and management teams. 12. Coordinates Plan of Safe Care (Maternal Health Council) meetings ensuring deliverables are met as indicated in state contract; attends Plan of Safe Care statewide meetings. 13. Ensures compliance with Federal, State, and local regulations and Comprehends mission policies, procedures and standards. 14. Completes all mandatory compliance, HIPAA and state and federal regulatory training as required. 15. Maintains appropriate degree of confidentiality. 16. Other duties as assigned. Qualifications and Education Requirements Bachelor’s degree, preferably with emphasis in Behavioral Science/Human Services, with 1-year relevant experience. Successfully pass pre-employment screenings and drug screen Physical Requirements Sitting, standing, walking Lifting and Material Handling: ability to lift 25 pounds. Special Requirements Proven ability to build relationships, trust and deepen connections with diverse people and groups. Must maintain a valid driver’s license with acceptable driving record throughout employment as driving is required; use of personal vehicle and maintenance of personal care insurance is required.

Assists the client in accessing services from other agencies/organizations. \\r\\n\\r\\n\\t\\t\\t5. Monitors client&rsquo;s progress towards treatment plan/goals. \\r\\n\\r\\n\\t\\t\\t6. Provided case consultation and crisis assistance as needed/required. \\r\\n\\r\\n\\t\\t\\t7. Participate in process improvement activities as directed by program needs/deliverables. \\r\\n\\r\\n\\t\\t\\t8. Complete service documentation and billing entries within 48 hours of service delivery. \\r\\n\\r\\n\\t\\t\\t9. Attend statewide Kentucky Moms meetings, trainings, symposiums, etc. as scheduled. \\r\\n\\r\\n\\t\\t\\t10. Enter all service data in the CDAR Data System including intake/baseline surveys and case management activities. \\r\\n\\r\\n\\t\\t\\t11. Fosters collaborative working relationships among members of the leadership and management teams. \\r\\n\\r\\n\\t\\t\\t12. Coordinates Plan of Safe Care (Maternal Health Council) meetings ensuring deliverables are met as indicated in state contract; attends Plan of Safe Care statewide meetings. \\r\\n\\r\\n\\t\\t\\t13. Ensures compliance with Federal, State, and local regulations and Comprehends mission policies, procedures and standards. \\r\\n\\r\\n\\t\\t\\t14. Completes all mandatory compliance, HIPAA and state and federal regulatory training as required. \\r\\n\\r\\n\\t\\t\\t15. Maintains appropriate degree of confidentiality. \\r\\n\\r\\n\\t\\t\\t16.
