Description
Carteret Management Corporation is seeking a bilingual RESIDENT SERVICE COORDINATOR to assist senior individuals and persons with disabilities living in Cathedral Townhouse , an affordable housing community located in downtown Jacksonville, to obtain needed supportive services from community agencies. This position offers full-time employment at a competitive salary with a strong benefit package including:

Responsibilities:
Provide general case management (including intake) and referral services to all residents needing assistance. Establish linkages with all agencies and service providers in the community. Set up a directory of providers for use by both project staff and residents. Refer and link the residents to service providers in the general community. Educate residents on service availability, application procedures, client rights, etc., providing advocacy as appropriate. Develop case plans in coordination with assessment services in the community or with a PAC. Monitor the ongoing provision of services from community agencies and keep the case management and provider agency current with the progress of the individual. Set up volunteer support programs with service organizations in the community. Help the residents build informal support networks with other residents, family and friends. Educate other staff on the management team on issues related to aging in place and service coordination to help them to better work with and assist the residents. Maintain detailed resident files.
