The SLP will work closely with the rehabilitation team to improve residents&#8217; ability to communicate effectively and safely manage swallowing disorders.</p><h2><strong>Key Responsibilities</strong></h2><ul><li>Conduct comprehensive <strong>speech, language, cognitive-communication, and swallowing evaluations</strong> to determine patient needs and establish therapy goals.</li><li>Develop and implement <strong>individualized treatment plans</strong> designed to improve speech articulation, language comprehension, cognitive function, and swallowing safety.</li><li>Provide therapy interventions for patients experiencing <strong>dysphagia, aphasia, voice disorders, cognitive impairments, and speech disorders</strong> resulting from stroke, neurological conditions, injury, or aging.</li><li>Monitor patient progress throughout therapy sessions and <strong>modify treatment plans</strong> as necessary to achieve optimal outcomes.</li><li>Educate patients and caregivers on <strong>home therapy exercises, communication techniques, and safe swallowing practices</strong>.</li><li>Maintain accurate and timely <strong>clinical documentation</strong>, treatment notes, and progress reports in compliance with facility and regulatory standards.</li><li>Collaborate with physicians, occupational therapists, physical therapists, nursing staff, and other healthcare professionals to coordinate patient care.</li><li>Participate in <strong>care planning meetings</strong> and interdisciplinary discussions to support patient recovery goals.</li></ul><h2><strong>Requirements</strong></h2><ul><li>Degree in Speech Language Pathology</li><li>Active VA state license&#160;</li><li>BLS certification</li><li>SNF experience preferred</li></ul>",
