Join us on this meaningful journey where your skills, compassion and dedication will make a remarkable difference in the lives of those we serve.</p><p>&nbsp;<strong>How you'll contribute&nbsp;</strong></p><p>A Licensed Practical provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.</p><p>Essential Functions</p><ul style=\"list-style-type: disc;\"><li style=\"margin-bottom: 0in;\">Demonstrates knowledge of medications and their correct administration based on the age and clinical condition of the patient</li><li style=\"margin-bottom: 0in;\">Performs patient care considering needs specific to the standard of care for patient’s age; includes geriatric and general inpatient rehabilitation patients</li><li style=\"margin-bottom: 0in;\">Under the supervision of the RN, assists with the assessment on all patients and reassessments as per policy.&nbsp;&nbsp;Identifies and initiates appropriate rehabilitation nursing interventions</li><li style=\"margin-bottom: 0in;\">Under the supervision of the RN, performs timely and accurate QI assessments</li><li style=\"margin-bottom: 0in;\">Carry out the plan of care as indicated by the patient’s needs and response to treatment; evaluates overall plan daily for effectiveness; updates interdisciplinary care plan accordingly based on changes in patient’s condition and individual needs</li><li style=\"margin-bottom: 0in;\">Interacts professionally with patient/family and involves patient/family in the formation of the plan of care</li><li style=\"margin-bottom: 0in;\">Demonstrates ability to adequately assess and reassess pain.&nbsp;&nbsp;Utilizes appropriate pain management techniques.&nbsp;&nbsp;Educates the patient and family regarding pain management</li><li style=\"margin-bottom: 0in;\">Other duties as assigned</li></ul><p style=\"margin: 0in 0in 0in 0.5in;\">&nbsp;</p><p>Additional Information</p><ul style=\"list-style-type: disc;\"><li style=\"margin-bottom: 0in;\">Strong interpersonal and documentation skills required.</li><li style=\"margin-bottom: 0in;\">Familiarity with rehab documentation systems is preferred.</li></ul><p>&nbsp;</p><p><strong>Why join us&nbsp;</strong></p><p>We believe that investing in our employees is the first step to providing excellent patient care. Additional requirements include:</p><ul style=\"list-style-type: disc;\"><li style=\"margin-bottom: 0in;\">Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.</li><li style=\"margin-bottom: 0in;\">Current state licensure as Licensed Practical Nurse.</li><li style=\"margin-bottom: 0in;\">CPR/BCLS certification.
