High school diploma or equivalent required.</p>\n<p><strong> </strong></p>\n<p><strong>Pay: </strong>$17.00/hr </p>\n<p> </p>\n<p><strong>Shift available: </strong></p>\n<p>10:00 pm - 6:30 am Tuesday - Saturday</p>\n<p> </p>\n<p>We are seeking a compassionate, detail‑oriented <strong>Patient Navigator</strong> to support our Intake & Needs Assessment team at our Pennsylvania Access Center. In this role, you will be the first point of contact for individuals seeking care and will play a vital part in guiding patients and their families through the admissions process. If you thrive in a fast‑paced environment, have excellent communication skills, and have a passion for helping others access the services they need, we’d love to meet you.</p>\n<p> </p>\n<h2>Responsibilities</h2>\n<p><strong>​</strong><strong>ESSENTIAL FUNCTIONS:</strong> </p>\n<ul>\n <li>​Complete activity assessment through patient observation and patient/family interview, obtain information on the patient’s needs, skills, interests and limitations for the development and implementation of the treat plan. </li>\n <li>​Plan, organize, direct and implement the activities within the treatment program to facilitate patient progress with treatment goals. </li>\n <li>​Plan and implement an activities program including but not limited to therapeutic leisure skills and activities, leisure education and leisure awareness. </li>\n <li>​Facilitate and lead activity sessions to improve patient mental and physical well-being. </li>\n <li>​Instruct patients in activities and techniques, such are sports, dance, music, art or relaxation techniques designed to meet their specific needs. </li>\n</ul>\n<ul>\n <li>​Develop treatment plans and implement activity interventions that meet patient needs and interests and help the patient achieve his/her treatment goals. </li>\n <li>​Plan and implement activity interventions that promote a safe use of leisure time. </li>\n <li>​Provide activity interventions to promote the development of social skills and leisure independence. </li>\n <li>​Modify activities to suit the needs of specific groups. </li>\n <li>​Direct and supervise activity staff in their implementation of the activity therapy program and services, if applicable.  </li>\n</ul>\n<ul>\n <li>​Demonstrate a positive, empathetic and professional attitude towards customers always.  When patient needs are not met, acknowledge and work to resolve complaints.  Recognize that patient safety is a top priority. </li>\n</ul>\n<p><strong>​</strong><strong>OTHER FUNCTIONS: </strong> </p>\n<ul>\n <li>​Perform other functions and tasks as assigned. </li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>​</strong><strong>EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:</strong> </p>\n<ul>\n <li>​Bachelor's Degree in Therapeutic Recreation, Recreational Therapy or related field required.   </li>\n</ul>\n<ul>\n <li>​Master’s degree preferred.  </li>\n <li>​Two or more years’ experience in recreation in a healthcare setting with knowledge of patient population served by the facility is required. </li>\n</ul>\n<p><strong>​</strong><strong>LICENSES/DESIGNATIONS/CERTIFICATIONS: </strong> </p>\n<ul>\n <li>​CPR and de-escalation/restraint certification required (training available upon hire and offered by facility). </li>\n <li>​First aid may be required based on state or facility. </li>\n</ul>\n<p> </p>\n<p>We are committed to providing equal  employment opportunities to all applicants for employment regardless of an individual’s characteristics protected by applicable state, federal and local laws.</p>\n<p> </p>\n<p>AHMKT</p>",
