    "requisitionDescription": "<div><div><div><div><div><div><p><strong>Job Title</strong></p><p>The Registered Nurse (RN) is responsible for managing the individualized patient care by promoting and restoring patients&#39; health through the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; and supervising assigned team members. The RN with this position is also responsible for clinical projects that will help close any gaps and ensure we are HRSA compliant.</p><p><strong>Communication</strong></p><p>Good oral and written communication skills are required for this position. &nbsp;Information must be exchanged using tact and persuasion appropriately. &nbsp;The person in this position must be able to communicate with the patients, staff, and others; and contribute to a positive environment. &nbsp;&nbsp;</p><p><strong>The ability to communicate in Spanish, Creole, or both is a plus.</strong></p><p><strong>&nbsp;</strong><strong>Key Responsibilities</strong></p><ul><li>Maintains a cooperative relationship among health care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods.</li><li>Establish and maintain processes to manage scope over the project lifecycle, setting project quality and performance standards, and assessing and managing risk within, and across, multiple projects.</li><li>Report project outcomes and/or risks to the appropriate management channels as needed&mdash;escalating issues as necessary based on project work plans.</li><li>Manages Chronic Care Management program to include on-site employees as well as remote third-party vendors. &nbsp;Devise monitoring of Remote Patient Monitoring and incorporate into best practices for patient and provider to have improved outcomes in chronic conditions.</li><li>Assures quality of care by enforcing adherence to state board of nursing and state nurse practice act requirements and to other governing agency regulations; measuring health outcomes against standards; making or recommending adjustments.</li><li>Develop algorithms for support staff to aid and follow up on provider clinical inboxes managing patient documents to include laboratory results, consultant reports, etc., including health maintenance education and follow-up.</li><li>Develop, implement, and regularly update clinical protocols and best practice guidelines in alignment with current evidence-based practices. &nbsp;Monitor and assess clinical practices to ensure adherence to established guidelines and standards.</li><li>Provide training and mentorship to support staff, including medical assistants, LPNs, and other clinical support personnel, on clinical procedures and best practices.</li><li>Conduct regular performance evaluations and provide constructive feedback to support staff regarding adherence to clinical guidelines.</li><li>Performs accurate charting in medical records in accordance with WFHC policies and procedures.</li><li>Attends educational offerings as appropriate.</li><li>Performs other duties and responsibilities as assigned.</li><li>May occasionally need to travel from one clinic to another for coverage.</li></ul><p><strong>&nbsp;</strong><strong>Education &amp; Training &nbsp;</strong></p><ul><li>Associate or bachelor&#39;s degree in nursing</li><li>Current RN licensure in good standing in the state of practice.</li><li>Must be cross trained in electronic medical records, scheduling software and Microsoft Outlook.</li><li>Understanding of standard medical practices and procedures</li></ul><p><strong>Experience</strong></p><ul><li>One to three years of progressive work experience as a nurse in a hospital or other health care setting.</li></ul><p>&nbsp;<strong>Requirements</strong></p><ul><li>Level 2 background clearance required</li><li>Proof of immunizations for MMR, HEP B vaccination, and either a history of chicken pox (varicella), or documentation of the varicella vaccine, current Tdap, and a PPD Test at time of hire.</li></ul><p style=\"text-align: center;\"><strong><em>&nbsp;</em></strong><strong><em>WFHC reserves the right to change or modify the job description, including but not limited to Major Responsibilities, Education, Certification, and Physical Requirements.&nbsp;</em></strong></p><p style=\"text-align: center;\"><strong><em>WFHC is an EOE.</em></strong></p></div></div></div></div></div></div>\n",
