Reporting to the APD, works with all other staff as part of a team.</p>\r\n\r\n<p><strong>Schedule:</strong></p>\r\n\r\n<p>Tuesday-Saturday 9am - 5pm</p>\r\n\r\n<p><strong>Essential Duties:</strong></p>\r\n\r\n<ul>\r\n\t<li dir=\"ltr\">\r\n\t<p>Assign cases to case managers and schedules work according to priority;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Maintain necessary records and supervision notes; prepare regular and special reports as required;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Oversee quality assurance of case management services to ensure compliance with Comunilife, and government policies and regulations;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Performs audits of client charts to ensure comprehensive services planning and accurate charting of client progress;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Ensures that the home visit policy and procedures is enforced, tracked and documented.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Provide training to new and current case mangers;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Speak to dissatisfied clients and handle difficult situations;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Receive regular supervision with the Assistant Program Director (APD) and inform of the general status of cases, staff performance, problems and trends;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Complete probationary and annual performance reviews for case management staff;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Develop and maintain cooperative relationships with ancillary agencies;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Prepare and submit all required program statistics and reports accurately and in a timely fashion;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Under the supervision of the Assistant Program Director, develop comprehensive group programming, in compliance with contract and agency requirements;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Coordinate and supervise efforts with case management and maintenance staff to ensure a minimum monthly occupancy rate of 95% is maintained;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Coordinate and supervise efforts to place new HASA clients in vacated units within a maximum of 30 days from referral;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Ensure that the initial and medical assessments are conducted, and that comprehensive service plans are developed that identify mutually agreed upon long and short-term goals;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Review and provide supervision in the development of case management service plans and ensure that reassessments are conducted on a quarterly basis (at minimum) to ensure that services are appropriate for the client&rsquo;s current needs;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Provide oversight and supervision in the delivery of mental health and alcohol/substance abuse services and referrals, including assistance in accessing primary medical care, assistance with adherence to medication regiments, activities for daily living, nutritional services, recreation programs, assistance in employment assessments, training, private sector placement, prevention with positives, and delivery of any other services necessary for clients to increase their capacity for independent living;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Ensure that all client linkage agreements are monitored and updated, as necessary;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Supervise and coordinate efforts to provide assistance in obtaining and maintaining all government benefits and services for which the client is potentially eligible. Ensure that all case managers assist clients who are incapacitated, unable to budget, and/or comply with their fiscal responsibilities by applying for Representative Payee Status, if necessary;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Ensure that all case managers seek, secure, and maximize all potential offsets with their clients, specifically by collecting the portion of income that is set aside by State and/or Federal authorities for rental payments from those clients receiving Supplemental Security Income, Social Security Disability, veterans benefits, or pensions that would be an offset against the amount that Comunilife would receive from HRA pursuant to the program budget;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Assist and supervise case managers efforts in assisting clients to locate an alternative housing option when a client&rsquo;s health so requires;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Complete, sign and monitor psycho-social assessments to assure completeness of documentation;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Conduct weekly supervisory conferences with each staff member of the case management teams to review case records and client&rsquo;s status and progress;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Contact&nbsp; HASA to arrange intake interviews for new clients and complete intake interview forms for each new client;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Provide the APD with information on the status of new referrals on a weekly basis;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Work closely with the APD to ensure a safe and adequate living space for residents;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Effectively communicate all building/unit concerns to APD in a timely manner;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Work with the APD in the development of guidelines for appropriate maintenance of units;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Attend all staff and community meetings</p>\r\n\t</li>\r\n</ul>\r\n",
