Patient Care\\n\\n\\n\\tProvide day-to-day clinical supervision for nursing and clinical staff supporting the Call center\\n\\tEnsure consistent execution of clinical workflows for:\\n\\t\\n\\t\\tCare management\\n\\t\\tRemote patient monitoring (RPM)\\n\\t\\tTriage and escalation\\n\\t\\tTransitional Care Management\\n\\t\\n\\t\\n\\tMonitor patient panels to ensure timely outreach, documentation, escalation, and follow-up.\\n\\tSupport adherence to clinical protocols, care pathways, and out-of-range monitoring standards.\\n\\tAssist with provider communication, including Telephone Encounters (TEs) and escalations as indicated.\\n\\n\\nStaff Supervision &amp; Support\\n\\n\\n\\tServe as a clinical resource and mentor for RNs, MAs, and support staff.\\n\\tConduct real-time coaching, skills reinforcement, and issue resolution.\\n\\tAssist with onboarding, training, and competency validation for new clinical staff.\\n\\tMonitor staffing coverage, workflows, and daily productivity; escalate concerns to Nurse Manager as needed.\\n\\tSupport performance improvement initiatives and corrective action when indicated.\\n\\n\\nQuality, Compliance &amp; Safety\\n\\n\\n\\tEnsure compliance with clinical policies requirements.\\n\\tSupport audit readiness through accurate documentation and workflow adherence.\\n\\tIdentify workflow gaps or quality risks and proactively escalate to leadership.\\n\\tReinforce patient safety standards and best practices across care settings.\\n\\n\\nOperational Support &amp; Reporting\\n\\n\\n\\tAssist with tracking and reporting operational metrics (e.g., RPM enrollment, outreach completion, care management activity)\\n\\tEnsure staff documentation is complete, timely, and aligned with program requirements.\\n\\tPartner with leadership on process improvements to enhance efficiency and patient outcomes.\\n\\tAssist with Timecard approval and Paid time off requests.\\n\\n\\nRequirements:\\n\\n\\n\\tStrong clinical judgment and ability to lead frontline nursing teams.\\n\\tExcellent communication and organizational skills.\\n\\tAbility to work independently while escalating appropriately.\\n\\tProven ability to coach, mentor, and support clinical staff.\\n\\tExperience in care management, chronic disease management, triage, or ambulatory care\\n\\tProficiency in EHR systems and Microsoft Office (Word, Excel, Outlook)\\n\\tActive Multistate Registered Nurse (RN) license.\\n\\tCurrent BLS for Healthcare Providers certification.\\n\\tKnowledge of federal, state, and local laws, statutes, regulations, codes, and standards related to the area of responsibility.\\n\\tKnowledge of the principles, concepts, and theories relevant to the assigned functional area.\\n\\tSkill in completing assignments accurately and with attention to detail.\\n\\tAbility to communicate clearly and concisely both orally and in writing in English, to various audiences.\\n\\tAbility to manage time, organize work, set priorities, meet deadlines, and follow up on work assignments with minimal supervision.\\n\\tAbility to work independently, and as a team, to complete daily activities according to the work schedule.\\n\\tWorking knowledge of Office 365 programs such as Excel, Outlook, Word, PowerPoint, etc.\\n\\n\\nPreferred Qualifications:\\n\\n\\n\\tBachelor&rsquo;s degree in nursing or related healthcare field.\\n\\t2+ years of experience in a supervisory or lead RN role.\\n\\tExperience in cardiology, population health, or value-based care programs.\\n\\tExperience supporting remote or virtual care teams.\\n\\n\\n&nbsp;\\n\\nAbout Dallas/Fort Worth, TX:\\n\\nDallas/Fort Worth, known as the Metroplex, offers a vibrant urban life and peaceful suburban living, making it an enticing place to reside.
