    "requisitionDescription": "<div><div><div><div><div><link href=\"https://cdn.jsdelivr.net/npm/[email protected]/css/froala_editor.pkgd.min.css\" rel=\"stylesheet\" type=\"text/css\">&nbsp;<link href=\"https://cdn.jsdelivr.net/npm/[email protected]/css/froala_style.min.css\" rel=\"stylesheet\" type=\"text/css\"><div class=\"fr-view\"><div><div><div><p><br></p><p><strong>Chaplain: Assists, educates, and provides support to patients and families with spiritual concerns.</strong> &nbsp;</p><p>SCHEDULE: VARIES AS NEEDED MON-FRI 8AM-4:30PM --- FILL IN FOR VACATIONS, VACANCIES, VOLUME &nbsp;</p><p><ins>JOB DUTIES/KNOWLEDGE</ins>: &nbsp;</p><p>Provides Chaplain services in accordance with the patients plan of care.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</p><p>Assists the Inter-disciplinary team to understand the significance of spiritual factors related to the patients care.&nbsp;&nbsp;</p><p>Assesses and reassesses spiritual factors in order to help the patient and family to cope with problems related to the patients life limiting illness.&nbsp;&nbsp;</p><p>Recognizes and utilizes community and family spiritual supports to assist with the patients plan of care.&nbsp;&nbsp;&nbsp;&nbsp;</p><p>Develops the plan of care with the Inter-disciplinary patient team and family to deal&lt;BR&gt;\\with spiritual difficulties experienced by the patient.&nbsp;&nbsp;</p><p>Develops, prepares, and maintains individual patient care progress notes with accuracy, timeliness, and according to policies.&nbsp;&nbsp;</p><p>Keeps abreast of Chaplain trends and knowledge. Participates with in-service programs. Attends and participates in Inter-disciplinary team meetings. Cooperates with appropriate staff to provide Chaplain services to patients and patient families.&lt;BR&gt;9. Provides education and training for Hospice of Lake &amp; Sumter, when requested and arranged by the Social Services Manager.&nbsp;&nbsp;&nbsp;&nbsp;</p><p>Participates in quality improvement program.&nbsp;&nbsp;&nbsp;</p><p>Participates in Hospice sponsored events.&nbsp;&nbsp;&nbsp;</p><p>Takes a leadership role in Cornerstone Hospice issues and events relating to the spiritual impact of life limiting illness.&nbsp;&nbsp;&nbsp;</p><p>Provides crisis intervention for patients and their families, when and where appropriate.&nbsp;&nbsp;</p><p>&nbsp;&nbsp;</p><p><strong><ins>Qualifications:</ins></strong> &nbsp;</p><p>&nbsp;<span style=\"font-size: 12pt;\">1</span> <span style=\"font-size: 12pt;\">Master of Divinity degree or similar graduate degree from a Seminary accredited by the American Association of Theological Seminaries, or the American Association of Theological schools, and a minimum of <strong>three units (3) of CPE</strong> <strong>or equivalen</strong>t (as recognized by ACPE, APC, NACC, NAJC, CPSP, or CPE-International).</span> &nbsp;</p><p><span style=\"font-size: 12pt;\">2.</span> <span style=\"font-size: 12pt;\">Five years of experience as either a pastor or chaplain in a Hospice House and/or Trauma Unit.&nbsp;</span> &nbsp;</p><p>3. Knowledge and Abilities:&nbsp;&nbsp;&nbsp;</p><p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;a) Demonstrated knowledge and commitment to the Hospice philosophy of care and the Hospice team concept&nbsp;&nbsp;&nbsp;</p><p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;b) Able to assess data reflecting the patients status and interpret the appropriate information needed to identify each patients requirements relative to their spiritual needs.&nbsp;&nbsp;</p><p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Ability to work comfortably in a non-denominational or multi-denominational environment.&nbsp;&nbsp;</p><p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;d) Knowledge of multi-cultural beliefs and practices.&nbsp;&nbsp;</p><p>4.
