Home Care Coordinator - Home Care Department REPORTS TO: Home Care Supervisor/ Clinical Director JOB SUMMARY: The Home Care Coordinator performs a variety of administrative duties in the Emerald Home Care office, including but not limited to; taking consumer orders, and scheduling Emerald Home Care employees to meet consumer needs. The Home Care Coordinator schedules field employees and provides administrative support for the Emerald Home Health home care department operations. The Home Care Coordinator understands basic clinical issues and conducts business diplomatically and maintains confidentiality by using effective communication skills. The Home Care Coordinator functions as the first point of contact to direct care workers. They may participate and provide written documentation on employee incidents to be used in counseling, as well as assist in the preparation of field staff evaluations. The Home Care Coordinator - May work with the Human Resources Generalist throughout the Interviewing and Orientation of new applicants. DUTIES AND RESPONSIBILITIES: Acts as telephone receptionist for the home care department and follows telephone guidelines set forth in the Emerald Home Health manuals. Prepares materials and assists the Human Resources Generalist with the interviewing and orientation process at the direction of the Home Care Supervisor. Receives consumer calls and inquiries. Coordinates field employee schedules with current caseloads, determines availabilities and schedules, Confirms assignments. Keeps consumer order records and employee information up to date. Makes in person visits to all home care and institutional consumers. Handles complaints tactfully and refers them to the Home Care Supervisor immediately. Assists in the preparations of employee evaluations under the direction of the Home Care Supervisor. Performs typing and other clerical duties at the direction of the Home Care Supervisor. Ensures consumer confidentiality. Receives and coordinates home care service requests. Performs Start of care visit including gathering and entering of information for Plan of care Assists as directed in gathering and processing information and referrals received electronically. Assists Home Care Supervisor and /or Private Duty Clinical Manager with preparing documentation for audits and surveys, both electronic and in person. Provides assistance as needed during audits and surveys. Inputs employee timesheets into HHA Exchange weekly or as received. Assists in managing the HHA Exchange dashboard. Sends and receives authorizations and re-authorizations: enters them into the electronic medical record, as needed. Verifies insurance data on prospective consumers. Runs Billing OPS 8 reports monthly, if needed. Maintains schedule calendars – keeping schedules flowing month to month. Runs and works through various reports at direction of Home Care Supervisor and/or Assistant Administrator to ensure daily operations continue uninterrupted, monitor compliance, and audit records. Tracks physician orders, recertifications, in-services, and supervisory visits. Maintain consumer clinical records/referrals in an orderly format in a secured area, as well as maintaining consumer electronic medical records. Assists the Home Care Supervisor and Assistant Administrator with general clerical tasks. QUALIFICATIONS/EDUCATIONAL REQUIREMENTS: College degree preferred from an appropriate institution. High school diploma or equivalent required. Minimum of two years’ work experience in a customer service capacity, home care setting, or medical staffing facility. Working experience in a high-pressure environment, including experience in adapting to changing priorities. Candidates must also have computer/ keyboard skills and proficiency in Microsoft Office software. PHYSICAL DEMANDS: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position without compromising administrative workflow. Office level work, Computer use, lift up to 30 pounds.
